Aural Rehab Assignment Ù 5

Salim Ahmad, a 3-month-old male, failed his newborn hearing screening but has no risk factors for hearing loss. If diagnosed with bilateral profound sensorineural hearing loss, he may benefit from hearing aids and potentially cochlear implants, along with parent counseling and various communication strategies. A long-term auditory-verbal therapy plan should focus on sound awareness, sound integration, and speech production, with regular assessments to track his auditory skill development.

Child case study

Age: 3 months

Gender: Male

: Background information and previous aural rehabilitation

Salim Ahmad had a normal delivery with no complications.
He had none of the usual risk factors for hearing loss, such
as low birth weight, family history of hearing loss, low Apgar
scores, or physical manifestations associated with a
syndrome. However, he failed the newborn screening test
that was performed with an otoacoustic emissions (OAE)
screening system. Before he left the newborn nursery, a
nurse explained the results of the screening test to his
mother, and encouraged her to bring him back for an
.audiologic examination within the next 3 months

:Family situation
Salim lives with both parents. His mother is a teacher and
.his father works as a news reporter. He is an only child

What tests might be performed by an audiologist to confirm

?the presence of a hearing loss

Auditory Brainstem Response (ABR) test 

This test measures the electrical activity In the auditory
.nerve and brainstem in response to sound

Auditory Steady-State Response (ASSR) test 

. measures the brain's response to specific frequencies

Suppose the audiologist found that Salim has a bilateral

profound sensorineural hearing loss. What should happen in
the next few months after diagnosis? (Please answer in
terms of listening devices use, candidacy for CI, parents
counselling, choice of communication mode and
rehabilitation approach)

Hearing aids
A hearing aid Is an electronic device that makes sounds
louder , best choice for Infants and toddlers are behind-the-
ear (BTE) style hearing aids.

Cochlear implants
Cochlear implants are made for children with severe to
profound hearing loss. Currently, the FDA does not allow
children under the age of 9 months to get a cochlear
implant. Infants who need cochlear implants often wear
hearing aids until their surgery takes place in the
. appropriate time

Parent counseling

Parent counseling is an important aspect of supporting a

child with hearing loss. It can provide guidance, education,
and emotional support to parents as they navigate their
child's hearIng journey. Counseling sessions can help
parents understand the Impact of hearing loss on their
child's development, learn strategies to promote
communication and language skills, and connect with
resources and support networks. It's essential for parents to
feel empowered and equipped to support their child's
.unique needs

?How Will My Child Communicate

Cued speech
An auditory and visual type of communication that uses
hand signals for different sounds. Different hand shapes in
different positions near the mouth help the child understand
what sound is being said
Early and regular use of hearing devices Is important if the
main goal is for the child to use listening and spoken
.language to communicate

Total communication
To support listening skills and spoken language while also
giving the child a way to communicate with family

Ongoing use hearing devices is strongly encouraged to help

with the development of the child's listening and spoken
.language skills

Rehabilitation approach

One example of a rehabilitation approach for Infants with

hearing loss is auditory-verbal therapy (AVT). AVT focuses
on developing listening and spoken language skills through
intensive therapy sessions. It involves teaching parents how
to create a rich auditory environment for their child, using
techniques like sweeping presenting sounds across the
array sequentially and Interpolation. The goal Is to establish
a map for delivering sound to internal devices and adjust
stimulus parameters for electrode activation. AVT aims to
restore audibility of speech and preserve a true
representation of sound intensity levels. It can be a highly
effective approach in helping infants with hearing loss
.develop strong communication skills

3- Suppose that you want to prepare an AVT long term plan

for Salim. Please list long term goals you believe it is
appropriate to work on with Salim and his family for the first
six months after CI in the areas of Audition, language and
speech.

After CI it’s Important to do aural rehabilitation, auditory

verbal therapy and family counseling , and it starts by
activating the cochlear implant after 6 week of the surgery
,and the programming journey begin

First, the child is learned to detect the sounds in order to

“ increase the “Awareness of the sounds

Second, he is learned to Integrate the sounds with its

meaning and actions
And then the child is learned to try to produce some speech
.sounds with the help of the speech therapist

The family has a great impact on the development and -

success of the implantation, by creating the ideal
environment for their child , and by using communication
strategies which facilitates the learning and language
acquisition for their child such as “ talking in a audible voice
for their child , talking in a slow rate , and keep up with the
. follow up sessions with the therapists of their child

Please list assessment procedures you believe it is

appropriate to use with Salim to track his development of
auditory skills during the first 2 years following CI and after
.that

To track the development of salim, some test should be

; performed such as

Since he is 2Years old , we can perform VRA to check his

hearing status after implantation, as well as words and
. sounds repetition
