This Trainee’s Record Book (TRB) is intended to serve as record of


accomplishment/tasks/activities while undergoing training in your respective institution. This will actually
become evidence that can be submitted for assessment and for whatever purpose it serves you. It is
important that all the required inputs are viably entered by both the trainee and trainer.

This TRB contains all the required competencies in TRAINER’S METHODOLOGY I. All activities
taken up during the training that used to be practiced and mastered are specified in this TRB. Please
expect your trainer to observe you every now and then using the Performance Criteria Checklist for each
activity as a guide.

Your trainer will likewise indicate his/her remarks on the “Trainers Remarks” column related to
the outcome of the task you have accomplished and have them confirmed by your trainer.

It is of great importance that the contents are written legibly on ink. Avoid any corrections or
erasures and please maintain the cleanliness of this record book on you will be subjected for disciplinary
action.

This TRB will be collected and submitted to GSAC, Maabay, Sibunag, Guimaras by your trainer.
