AFFIDAVIT

I, SUBHASH RAO, aged about 65 years, son of Mr. I. Vasudeva Rao,


presently residing at #1/S/19/1474-23, Flat No. 403, Adira Apartment,
Urwa Marigudi Road, Ashoka Nagar Post, Mangaluru, Dakshina
Kannada District, Karnataka State – 575 006, do hereby solemnly affirm
and state on oath as follows:-

(a) I say that, I have a DMAT account with Canara Bank Securities
Limited (formerly Gilt Securities Trading Corporation Limited) under
name SUBHASH RAO I. as per client ID 20383669.

(b) I say that, by inadvertent alphabet/letter “I” was included in my


name at the time of opening of the above said DMAT account which
stood for “IDDYA” which is family name and the same is reflected in
my father’s name in the PAN card issued by Department of Income Tax
bearing No. ABVPR0829A.

(c) I say that both as “SUBHASH RAO I.” and “SUBHASH RAO”
refers to me only.

(d) I say that in my PAN card issued by Department of Income Tax


bearing No. ABVPR0829A, my name has been correctly mentioned as
“SUBHASH RAO”.

(e) I say that even in the Aadhar card bearing No. 4422 9462 6158 my
name is mentioned as “SUBHASH RAO”.

(f) I say that, as by inadvertent alphabet/letter “I” was included in my


name, I have changed my name from SUBHASH RAO I. to SUBHASH
RAO and all identity cards/documents issued by various departments/
statutory authorities my correct name is mentioned as stated above.

(g) That, I am producing this Affidavit for the purpose of evidence for
my Name and for the purpose of correction of my name in the DMAT
account as per client ID 20383669.
(h) I say that, I shall at all times hereafter in all records, deeds and
writing and in all proceedings dealing with transactions, private as well
as under all occasions what so ever be known as only as “SUBHASH
RAO”.

(i) That what is stated above is true and correct to best of my


knowledge, information and belief. Nothing material has been concealed
there from.
