I, , hereby give my consent to the organization for the processing of my Personal Data for the
purposes outlined in this form. I understand that the organization will require my Personal Data
(as defined below) for various purposes during the course of my employment and in certain
circumstances even after cessation of my employment.
In light of the foregoing, I hereby agree to provide and shall provide during the course of my
employment with the organization, such "Personal Data" (which shall mean any information
relating to me, directly or indirectly, in particular by reference to an identification number or to
one or more factors specific to my physical, physiological, economic, cultural, or social identity)
as requested by the organization.

I hereby provide my express consent for the collection and use of the Personal Data provided
by me in accordance with the terms of this Consent Form as provided hereunder.

I understand that my Personal Data will be retained for as long as necessary to fulfill the

implements appropriate technical and organizational measures to protect my Personal Data
against unauthorized access, alteration, disclosure, or destruction.

I understand that I have the right to access, correct, modify, or delete my Personal Data, and I
also have the right to withdraw my consent at any time by contacting the organization at
privacy@startek.com.

The terms of this Consent Form shall be deemed to form an integral part of the Employment
Agreement and shall be read as if they are specifically incorporated therein. I hereby consent to
the processing of my Personal Data for the purposes specified above.
