I, Sulochana Bhaskar Jambusaria, aged: 62 years, residing at 19808 Kittridge


St, Winnetka, CA 91306, am a U.S. citizen. I am currently working as a Senior
Billing Representative at United Healthcare (Optum), for the past 8 years.

I am writing to invite my nephew, Sagar Sureshkumar Thakker, along with his


family (spouse and two children), to visit the United States. The purpose of their
visit is for tourism and to spend time with our family. Their tentative stay is
planned from 12th November 2024, for approximately two weeks.

The family members visiting will include:

• Sagar Sureshkumar Thakker, Passport # Y1371351


• His spouse, Vasundhara Shanker, Passport # Y5692013
• Their daughter, Trishika Sagar Thakker, Passport # U6295033
• Their daughter, Devyani Sagar Thakker, Passport # W4902648
During their stay in the United States, they will be staying with me at my
residence. I will assist with their local arrangements and provide any additional
support needed during their visit.
I am enclosing a copy of my IDs (passport and driver’s license) as required for
the visa application process.
