SESSION SUMMARY
The Recurrent training program is a 6 month’s training cycle which consist of:
 Day 1 – Ground recurrent training
 Day 2 – LOFA (Line Operation Flight Assessment) and PPC (Pilot Proficiency Check) on
Full Flight simulator (FFS) device
 Day 3 – LOFT (Line Operation Flight Training) and SPOT (Special Purposes Operation
Training) on Full Flight simulator (FFS) device

Ground recurrent class has a purpose to enhance pilot’s knowledge both in technical or non-
technical side appropriate with the session highlight. The class also facilitate pilot for discussion
for next day simulator session. Technical quiz is also required to review pilots aircraft system’s
understanding and as a requirement to continue on the next simulator sessions with minimum
score of 75 %

As the EBT (Evidence Based Training) start to be implemented in Lion Group training system,
The LOFT session is provided to correct and to enhance pilot competencies based on LOFA
and PPC results. It is a review of general procedures, normal, non- normal, supplementary
procedures, and some of emergency procedures. Training data from previous semester (July –
December 2024) indicates that the top 5 competencies need an enhancement are:

 Situational Awareness
 Flight Path Management - Manual
 Application Of Procedure
 Application of Knowledge
 Workload Management

The check is 3:50hr duration. A mandatory rest of 10 mins is required during this session. The
examiner may exercise discretion to determine when the rest time is allocated during the session.
All check/tests must be conducted as per the attached profiles.

LOFA – PPC program is intended to revalidate both Pilot ‘s aircraft type rating and Instrument
rating. No instructor intervention during the session. However, some “freeze” and reposition during
the session is still possible with the examiner discretion to save time without interrupt the exercise
being flown. In some exercises, where more than one exercise option is available, instructor is
free to pick one of those. Both options are equal in level of difficulties and equal in focus
competency.

During LOFT session crew will be expected to respond to instructions and situations as they would
on a line flight, and to use flight deck resources available (autopilot, auto throttle, etc.) LOFT is a
training session. Instructor must be fully involved at all times throughout the session. These
exercises should be fully facilitated by the instructor, timely intervention is needed to ensure
positive outcome. Exercises should be debriefed and repeated if crew does not achieve an
acceptable standard. Instructor will act as ATC, Flight Operations, Ground Engineer, or flight
attendant as needed.

For SPOT session trainee will practice some normal and non-normal maneuvers. Instructor will
facilitate a repositions, instructions, constructive critiques, and suggestions to enhance trainee
performance.

DEBRIEFING & CORE COMPETENCIES

A fundamental concept behind competency-based training and assessment is the requirement to


teach an exercise until the student displays competency. This requires the instructor to be able to
diagnose the fault made by the student and provide guidance on how to resolve the problem. One
particular exercise may give the opportunity for a pilot to develop their skills in a number of
competency areas and instructors should be aware of these competencies and address any weak
areas if they arise.

Competency based training requires the completion of certain tasks and competent flight crew
members should be able to complete tasks reasonably expected of them under achievable
conditions. Competency based training is about helping pilots improve from a minimum
standard of performance.

If the crew/candidate has passed the check they should be told immediately at the start of the de-
briefing and complimented if they have done well. The aim of a well-structured debrief is for the
crew to learn and improve their performance over the long term; understanding what happened
and why. They should leave the room with a very clear understanding of their strengths and
weaknesses that they displayed during the session. Facilitate a fair and unbiased debriefing of
the candidate based on identifiable factual items. Encourage candidates to analyse their own
performance, this relies upon the student’s own perception and willingness to perform well and to
maximise their learning from the simulator session

The main goal of an effective competency-based assessment system is to identify and correct
errors in pilot performance during a training exercise. With the errors observed and recorded, the
instructor should begin a process of root cause analysis by using the core competencies to identify
the underlying deficiencies leading to the failed exercise. This may be due to a failure in a number
of areas, however, it is more likely to be one or two competencies that have caused the initial poor
performance which has then led to a manifestation, observable by the instructor, as a number of
exceedances.

RECORD KEEPING

After each session of simulator, it is mandatory for Examiner or Instructor to fill up the LITES after
the end of session and hand over to the trainee. Record keeping is vital in any form of training or
checking. Their value lies equally in keeping a record of the past and identifying and refining goals
for the future. The comment column is not an “advice” to the trainee but it is more important to
record the facts. The phrase such as “Standard Progress” should be avoided. The text entered in
the comment box must reflect the performance during the training or checking event regardless
of the outcome of the debrief. Accuracy is essential for management to determine trainee
performance, what follow up need to be conducted after the session. This also helpful for other
instructor when conducting the corrective or enhancement session

GRADING

Grading for competency as the combination of knowledge, skill, and attitude is required to perform
a task to prescribe standard under a certain condition which are based on technical and non-
technical skills

UNSATISFACTORY or FAIL is defined as follow:

1. Should any Score 1 be given in the training form.


2. Score 1 is also given whenever trainee shows weakness/minimum acceptable
performance in one same competency on more than two flight phase/manoeuvre.
3. Should any Score 2 be given on more than two difference competencies.

Score (1):
UNSATISFACTORY/ UNSAFE SITUATION This item or event results in fail, if any repeat is still
below minimum standard. The trainee requires debrief, and comment must be filled in the
Training and Assessment Form. The Pilot did not apply knowledge procedures, communicate,
manage the automation, lead or work as a team member, solve problems or make decisions,
manage the workload and situational awareness correctly or inadequate; by RARELY
demonstrating ANY behavioral indicators when required, which resulted in an unsafe situation.
Remedial/Corrective Training is required for any graded 1 competency.

Score (2):
MARGINAL/ MINIMUM ACCEPTABLE this item or event is repeated with developing to
satisfactory after repeated. The trainee requires debrief and comment must be filled in the
Training and Assessment Form. The pilot applied knowledge procedures, communicate,
manage the automation, lead or work as a team member, solve problems or make decisions,
manage the workload and situational awareness was not adequate or at the minimum
acceptable level by only OCCASIONALLY demonstrating SOME of the behavioral indicators
when required, but which overall did not result in an unsafe situation.

Note: Remedial/Corrective Training is required for: - More than two grades 2 in a same
competency. - More than four grades 2 in different competencies.

Score (3): SATISFACTORY/ ADEQUATE


This item or event is satisfactorily performed with rooms for improvement after the session. The
trainee requires DEBRIEF and comment is NOT to be filled in the Training and Assessment
Form. The Pilot applied knowledge procedures, communicate, manage the automation, lead or
work as a team member, solve problems or make decisions, manage the workload and
situational awareness was adequately by REGULARLY demonstrating MOST of the
performance indicators when required, which resulted in a safe operation.

Score (4): GOOD/EFFECTIVE


This item or event is good and effectively performed on safety and operations. The trainee does
not require debrief and comment is NOT to be filled in the Training and Assessment Form. The
Pilot applied knowledge procedures, communicate, manage the automation, lead or work as a
team member, solve problems or make decisions, manage the workload and situational
awareness effectively by REGULARLY demonstrating ALL of the performance indicators when
required, which enhanced safety.

Aircraft will be positioned on Parking Stand 04, start from preliminary preflight. Allow trainee to get
comfortable with cockpit and FMC CDU setup. Pre departure briefing should be done in pre
simulator briefing in Class.

Session starts when Cabin Crew report passenger complete and ready for close door, once FO
announce “Close Door, Arm Slide and Crosscheck”. Session must be conducted as real as normal
operation. Instructor will act as ATC, Flight Attendant, Ground Personnel, etc.

Instructor intervention (if required), must be limited only to maintain the session within the time
constraint, or for the purpose of rectifying sim device issues.

EXERCISE OBJECTIVES:
 Able to identify an engine start abnormality and apply the appropriate approved
operating procedures.

 Aircraft position on Gate 04. Trainee set up the aircraft from preliminary preflight, allow
trainee to get comfortable and review all the aircraft configuration and procedures.
 Trainee should complete Preflight Checklist.
 Instructor call the cockpit as cabin crew, report “Captain, Pax 180 all adult complete, all
documents on board, and ready to close door.”
 These actions indicate LOFA session start and Instructor should act only as ATC, Flight
Attendant, Engineering, Operation Personnel, etc. Some repositions are still possible
under instructor discretion to save time, Instructor should call “I Have Control” before
repositioning aircraft.
 After doors are closed, FA report to Flight Crew and aircraft ready for pushback and start,
Trainee should complete Before Start Procedure and Before Start Checklist.
 Give clearance the aircraft to pushback heading NORTH for runway 18.
 Start engine during push back with several malfunction below.
 OPTION A: With Start Valve Remain Open. Trainee shouldn’t perform immediate action
to abort the start. Trainee should be aware to this non-normal start and carry out NNC.
Observe trainee when read “START VALVE OPEN” NNC. After finish the checklist,
trainee must evaluate the problem using MEL.

 OPTION B: During start, N2 increases really slow and creates Hung Start. Trainee should
perform action to abort the start. Trainee should be aware to this non-normal start and
carry out abort engine start procedure. Observe trainee when read “Aborted Engine Start”
NNC. After finish the checklist, trainee must evaluate the problem using MEL, since this
malfunction does not mention in MEL, it considers No Go.
 While giving any malfunction above, Instructor may give any distraction as Cabin Crew
and ATC, these action to observe trainee’s Workload Management.
 Observe trainee performs procedure based on Configuration Check procedure and
assess their Decision-Making concept. Once completed, inform trainee that all start
malfunctions are cleared and reset. Instructor reset all malfunction and system, continue
Before Taxi Procedure.
 Then taxi normally to holding point runway 18
 Capture flight when aircraft complete line up runway 18

 After the aircraft passing FL 200 ,Instructor shall give pressurization problem
 OPTION A: CABIN LEAK (Set 90%). This malfunction will make cabin rate climb to be
excessive and CABIN ALT will keep increasing. Observe trainee anticipation regarding
this malfunction and further action of the trainee. If time permit before cabin altitude
warning horn sounds, let trainee conduct the NNC of “AUTO FAIL or Unscheduled
Pressurization Change.
 OPTION B: CABIN LEAK (Set 10%) then Instructor acted as flight attendant shall give
information to the cockpit that there is a loud noise near the aft cabin. And 1 minute after
trainee initial reaction, Instructor shall activate AFT CARGO DOOR OPEN
 Assess trainee how they manage this event using the FFA strategy, and how they make
decisions using FORDEC. With this condition, emergency descent memory item shall be
performed. Following QRH , SEP and OM.A
 Verify that the trainee performs proper escape maneuvers, considering MEA and grid
mora.
 After emergency descent, observe trainee how they manage post decompression event

 After a while, contact the cockpit and says : “Captain, no doctor or nurse
onboard the aircraft at this moment, until now, the passenger is still
unconscious, I suspect he had an heart attack and need immediate medical
assistance”.
 Assess trainee how they manage this event using the FFA strategy and how they
make decisions using FORDEC. With this condition, diversion should be made
based on the weather conditions below:

procedure regarding approach for Runway 18 MDC.
 Based on weather condition during the approach, the approach light
should be insight at approximately 1000 feet AGL (depends on
device visual system).
 If crew unable to complete the approach to land and execute a go
around due to Unstabilized approach, maximum grade 3 should be
given in flight path manual competency.
 After the aircraft safely landed, the exercise is completed.
 Recall the “outbound-holding” snapshot and repeat the exercise for
other pilot.
 This is the end of LOFA exercises.

 Below MDA until 100 AGL PM should only look inside the flight instrument to closely
monitor the flight path and other instrument to make sure stabilized approach can be
maintained
 PF will keep visual reference insight while maintaining the proper path from the flight
instrument.
 Many cases crew did not realize that they de-stabilize the approach just after the runway
insight due to fixated on seeing outside and trying to locate the runway, or poor in
scanning.

LOFT – (SCENARIO) – RHS/FO as PF


LOFT is a training session. Instructor must be fully involved at all times throughout the session.
At any stage throughout the exercise questions from the crew should be encouraged. These
exercises should be fully facilitated by the instructor, timely intervention is needed to ensure
positive outcome. Exercises should be debriefed and repeated if crew does not achieve an
acceptable standard. The exercises in LOFT and SPOT will be instructor led with emphasize on
training the crews the correct handling skill and other soft skill associated with training objectives.

Pilot must present the LOFA / PPC (SIM DAY 1) LITES copy for being evaluated by instructor if
any competencies are needed to be reviewed, corrected, or enhanced. Pilot also required to
present the Recurrent Ground completion certificate and TKC result to instructor as a proof of his
readiness. Total FFS time is 03.50 minutes where the last 20 minutes is allocated for corrective
training from previous PPC session, if required, or for crew enhancement in any training subject.

All crew briefing will be completed during the pre-sim briefing as well as performance calculation
and MEL/DDG procedure review.

Cockpit Preparation

Aircraft is positioned at gate 4 with APU running. Captain and FO preflight are completed, FMC is
loaded, and passenger boarding is in progress. The session starts after cabin call “Passenger and
document are Complete, ready to close door”.

On January 1st, 2024, Lion air and Batik Air start to use new SOP for standard line operation
procedure, therefore instructor needs to observe how crew implement this new procedure during
the recurrent session. The focus competencies during this phase are Application of Procedure.

 Instructor should complete the cockpit preparation up to Before Start Procedure.


 Load the FMC Flight plan including the performance data.

where sudden change of wind can reduce performance capability in critical situation at low
altitude. Trainee should be able recover the performance of aircraft by execute windshear escape
maneuver as quick as possible. This discussion should be carried out during pre-simulator

 At FL 220 set APU OVERSPEED


 Trainees should conduct a Non-Normal Checklist (NNC) and assess how they would
manage this event by utilizing FFA strategies and applying the FORDEC. Under these
circumstances, a diversion should be made in accordance with the Quick Reference
Handbook (QRH) Checklist Instructions
 Distress or Urgency call to ATC should be made as soon as possible they identify this
event.
 Provide weather as below:
WAAA: RW03 110/12 5000 -RA OVC011 24/24 1005
WALL: RW27 150/14 800 TSRA OVC015 25/25 1006
WAFF: RW15 150/14 4000 -RA OVC012 25/25 1007

 After decision has been made, give trainee clearance base on STAR on related airport
below:
WAAA: VENRU 1D RW03
 RTB to MAKASSAR should be made due to weather condition below minima at the other
airport.
 Observe trainee during modification FMC CDU, verify trainee modify Reserve Fuel in FMC
based on Inflight Fuel Policy in OM A.
 Assess their flight management during diversion.
 The approach used in WAAA is ILS RW03 . Instruct the crew to proceed to OLVAS point.
 Assess trainee how they conduct precision approach. If any procedure deviation during
this phase, Instructor shall brief trainee on Post Simulator Briefing.
 The session is ended after aircraft safely lands

 Manages the flight path safely to achieve optimum operational performance and avoid
from terrain contact
 Crew able to demonstrate good crew coordination during and after the maneuver

Exercise

Allow the crew time to get comfortable with seat position, aircraft configuration, and aircraft
position. As this is a Special Purposes Training instructor should be fully involved and make input
to ensure a positive training outcome

 Aircraft is positioned to “S05:21.8 E120:09.0” at 5000 ft heading 280


 Reduce ZFW to 55.000
 GPWS aural “CAUTION TERRAIN” will come first at approximately 20-40 seconds from
projected impact with terrain. FOR TRAINING PURPOSES, TRAINEE DOES NOT NEED
TO REACT TO THIS CAUTION.
 When the aural “PULL UP” comes up, accomplish the GPWS escape maneuver (FCOM
MAN 1.6)
 Exercise consider complete when aircraft visually clear of terrain , or passing the GRID
MORA when in IMC, pitch reduced and accelerates

 Instruct trainee to switch pressurization mode to manual refer to FCOM Supplementary

 Instructor give them ATC clearance “Climb via SID to FL 300 Clear for take off”
 Observe how trainee manage their workload and keep their situational awareness high
during conduct this operation.
 Instructor may give trainee guidance if necessary
 Exercise consider complete after trainee fully understand Manual Mode Operation

This is the Evidence - Based training where the number of improper go-around events was
increased, especially in B737 aircraft. Sometimes improper go-around can lead into UAS
(Undesired Aircraft State) and TCAS-RA event.

The following scenario is to see how trainee demonstrate Application of Procedure, Flight Path
Management – Manual, Flight Path Management – Automation and Workload Management.

 Reset system and Malfunction and reposition aircraft to 6 miles Final Runway 21 at
NIGHT with Flaps 40 configuration.
 Engage both autopilots .
 At minimums, Instruct crew to Go-around, to OLVAS CLIMB TO 4000 FT
 From the go-around demo in previous matrix, in any configuration and weight, the pitch
and thrust required during go around normally around 10º and 90% N1.
 When a go-around is initiated, guide trainee to advance thrust to reduce GA thrust. It will
help pilot to achieve 1000 – 2000 FPM vertical speed. Based on Automatic Go Around
explanation on FCOM System Description.
 When go-around is initiated from manual approach (AP and ATS OFF), engaging the
autothrottle ON after go-around will move the thrust lever to maximum GA N1 limit. Pilot
need to be aware since it will instantly affect the vertical speed.
 Guide trainee with the correct go-around thrust setting and go-around pitch attitude.
Thrust increment will instantly affect the pitch to go UP, re-trim attitude as necessary.
Include this discussion when explaining the importance of scanning the instrument and
knowing the correct go-around attitude.
 Exercise finish after PF call “After Take Off Checklist.”
 Repeat the exercise from 3 miles with following aircraft configuration:
o Flaps 40 / GW 70 Tons
o Flaps 30 / GW 55 Tons
o Flaps 30 / GW 70 Tons Note slight change in pitch and Thrust setting for each
configuration.
 Go-around procedure and Go around automation already discussed during the recurrent
ground session. Make sure crew have good understanding before continuing to the next
exercise.
 Exercise is complete after flaps are Up.

OBJECTIVES:
 Pilot demonstrates procedural knowledge before entering ETOPS segment, and
using available resource able to identify the situation and consider appropriate
options

EXERCISES:

 Recall FMS data (MDC CGK) from previous save with some modifications to ZFW: 59.6
and Fuel: 8.1
 Position the aircraft to BIKPI point (S0036.8 E118 28.3) enroute on T14 at FL 320. This
will bring the aircraft to approximately 5NM before ETOPS entry point (E.EN)
 Invite trainee to put the following point into the FMS flight plan:
o ETOPS Entry point
o ETP
o ETOPS Exit point
 Exercise start after all point are entered in to the FMC
 Observe trainee when comparing the Flight Plan Fuel with the actual fuel state
 Prior to ETOPS Entry point, ensure trainee complete the ETOPS briefing card.
 Fail the APU when crew try to start the APU before the E.EN (MALF 49,3)
 APU running is mandatory for the ETOPS segment. Flight can not continue to enter the
ETOPS segment. The only suitable airport en-route is WARR which is out of the 60
minutes flying time, the flight should be diverted back to WAMM
 The exercise complete after trainee decide return back to Jakarta and FMS modification
are completed.
