AFFIDAVIT OF LOSS

I, MARCO TRACY M. PANINGBATAN, of legal age, Filipino and a


resident of Barangay Poblacion 2, Sta. Cruz, Occidental Mindoro after
being duly sworn to accordance with law, hereby depose and say that:

1. I was a holder of a Non Professional Driver’s License issued by LTO,


upon issuance of said Driver’s License I kept it inside my wallet for
safekeeping;

2. On June 6, 2025, I went home from Brgy. Poblacion 2, Sta. Cruz, to


Brgy. Balansay, Mamburao, Occidental Mindoro boarded by my
motorcycle, I then have with me my wallet containing said Driver’s
License in my pocket;

3. When I arrived at my destination, I noticed that my wallet


containing said Driver’s License is no longer on my pocket;

4. I looked for the said wallet along the route I took but I could no
longer find it. Earnest efforts were made to locate the same but to
no avail. Considering the foregoing circumstance, it is my belief that
I may have dropped the said wallet and thus, they can already be
considered lost and beyond recovery;

5. I have executed this affidavit to attest to the truthfulness of the


foregoing and for all legal intents and purposes this may serve.

IN WITNESS WHEREOF, I have hereunto affixed my signature this 9 th


day of June, 2025 at Mamburao, Occidental Mindoro.
