AFFIDAVIT

I, SHRUTHI, W/o. Ramesh, Aged about 40 years, R/at. 4-13, Netla house,
Ajjibettu Village, Mangalore, D.K., do hereby solemnly affirm and state as
follows:-

1. I say that I am the Deponent herein and I reside at the address mentioned
above.
2. I am the holder of Indian Passport No. _______ issued on 20-07-2024 at
Bengaluru.

1. In said passport my name is stated as SHRUTHI RAMESH.


2. In all other certificates including Aadhar card my name is stated as
SHRUTHI.
3. SHRUTHI and SHRUTHI RAMESH stated above is one and the same person.
It denotes me.
4. I say that, the above mentioned both name are my name.
5. I say that, I am ready to produce all the required documents necessary to
prove it.

All the facts stated above are true.

contents of this affidavit having been read over and explained to the Deponent in
