CHILD REGISTRATION FORM (CRF)


One form is completed for each child being registered in BEP Sponsorship Program. When more
than one child is registered in the same family, complete separate forms for each child. Each child
must have a five digit Sponsorship Identification Number prefixed with the Country Code 101. Each
child’s initial registration form will be renewed every two years as they grow up, change physical
feature and probably residential address as well as school too.

Section 3: Child and Family Description
Describe the child in a way reflecting his/her real living situation. What is his/her personality
like? What is his/her interest? Likes and dislikes? Hobbies and future aspirations? This
information will describe the child and family to the potential sponsor and, the more it is
specific, the better the sponsor will be informed about the child and family background.

Alieu Jatta is staying with both parents, a regular student and love coloring. His mother is the
school Nanny (cleaner) while his father is a Masonry. The income for both parents are less than
GBP75 and not enough to sustain the family. Alieu is a very smart kid if assisted he will be able
to complete his education and will be a great benefit to him and his family.

the Brufut Education Project Child Sponsorship scheme. I permit my child to received
correspondence from his or her sponsor and to write letters to his or her sponsor. I agree that all
correspondence to and from this sponsor will be done through the staff of Brufut Education
Project and representatives of the project in diaspora and be opened and reviewed for
appropriateness and confidentiality by the staff of the project and/or representatives in diaspora. I
also give permission for a photograph of my child to be sent to the sponsor and to be used in
BEP in advertising media including Television, Mail and Internet.
