AMTEC WAIVER FOR MACHINE SPECIFICATIONS

FoodAgri Innovation & Technology House OPC fully understand that any
(Company)
document indicating the machine specifications is a requirement of the AMTEC testing
and evaluation process. This is in pursuance to AMTEC Terms and Conditions No. 1: “The
applicant shall submit the duly signed AMTEC-OP-F1 (Agricultural Machinery Test
Application Form) to the Information Office of AMTEC personally, through a courier, or
through email. The application form must be accompanied by any document (i.e.
operator’s manual, brochure, etc.) containing the machine specifications.”

As to prevent any delays, I allow AMTEC to release the corresponding Test Report/s
of the machine/s even if we fail to provide the aforementioned document three (3)
weeks after the testing proper.

By signing and submitting this Waiver Form, I hereby understand and agree with
the terms enclosed in this document. I fully release AMTEC and its Staff from all liabilities
in cases of data deficiencies and damages brought upon our noncompliance with the
aforementioned terms.
