Duly Accomplished PDS (CSC Form 12, Revised 2017) with work experience sheet , if applicable
Original copy of scholastic/academic record such as but not limted to Transcript of Records (TOR) and Diploma including
completion of graduate and post graduate units/degrees, if applicable (____Bachelors ____MA ____Doctoral)
Certified, Authenticated and Verified Transcript of Records (CAV) from CHED-if from private school ( _____MA _____Doctoral)
Notarized self-certification that the applicant officially enrolled/personally attended and obtained his/her units/diploma during the
