Work-From-Home (WFH) / Hybrid Work Agreement

This agreement supplements your employment contract with FPS Tech Philippines, Inc.

As part of your employment, you have been assigned to a Work-From-Home (WFH) or Hybrid
work setup. This agreement outlines the expectations and responsibilities associated with this
arrangement.

1. Connectivity, Work Performance, and Onsite Recall

Employees must ensure a reliable internet connection. The company reserves the right to recall
employees to work onsite based on performance concerns or operational requirements.
Persistent connectivity issues may require the employee to work from the office or an alternative
location with stable access.

This provision applies equally to employees under full WFH or hybrid work arrangements.

2. WFH Conduct and Productivity Guidelines:

Employees under WFH or hybrid work setups must:

●​ Maintain a dedicated, distraction-free workspace.


●​ Ensure a stable, high-speed internet connection and a back-up connection in case of
interruption.
●​ Follow regular working hours and remain available during scheduled times.
●​ Communicate effectively via designated company channels.
●​ Use company-approved devices, VPNs, and tools to ensure data security.
●​ Take scheduled breaks for productivity and well-being.
●​ Strictly avoid using personal devices for work-related activities.
●​ Set up an ergonomic home workspace to promote comfort and productivity.
●​ Uphold the same professional and performance standards as employees working onsite.

3. WFH/Hybrid Location Approval

Employees must obtain prior written approval before relocating their residence or primary
work-from-home location. Requests must be submitted at least 30 calendar days in advance to
the immediate supervisor, HR, and IT, and must include:

●​ New proposed address


●​ Reason for change
●​ Impact on work availability or connectivity
Approval will be granted on a case-by-case basis depending on business continuity and IT
supportability.

4. Compliance and Enforcement

Failure to comply with WFH/hybrid work guidelines—including connectivity, conduct, location


approval, or data security—may result in revocation of work-from-home privileges and/or
disciplinary action consistent with the company’s Code of Discipline.

5. Agreement and Acknowledgment

By signing this form, I confirm that:

●​ I have read, understood, and agree to follow the FPS Tech Philippines Inc. WFH/Hybrid
Work Guidelines.
●​ I acknowledge that I am covered by the same responsibilities, benefits, and company
policies regardless of work setup (onsite, hybrid, or WFH).
●​ I understand that this arrangement is subject to ongoing review based on company needs
and individual performance.
