Triage Classification

This document discusses different triage classification systems used in pre-hospital and hospital settings. It describes the START method, a simple triage approach that assesses ventilation, perfusion, and neurological status to classify patients into immediate, delayed, or deceased categories within 60 seconds. The document also outlines the Patient Acuity Category Scale, which ranks patients from 1 to 4 based on the severity of their condition and their need for urgent attention and resources. Proper triage classification is important to direct patients to the treatment level required by their medical needs.

TRIAGE CLASSIFICATION

Triage Classification 1
OBJECTIVES
To describe the different pre-hospital and
hospital classification systems
To describe the Patient Acuity Category
(PAC) Scale

Triage Classification 2
Triage Classification
Based on:
knowledge
available data
current situation

Triage Classification 3
3 Categories Triage System
Original form of classification:
Highest priority
Second priority
Lowest priority
Problems:
too generalized
not specific
too much subjectivity

Triage Classification 4
4 Categories Triage System

Highest priority Immediate / Class 1 / Severe / emergent

High priority Secondary / Class 2 / Moderate / Urgent

Low priority Delayed / Class 3 / Mild / Non-Urgent

Deceased Probable death / Class 4 / Class 0

Triage Classification 5
START METHOD
(Simple Triage and Rapid Treatment)
Useful for large number of casualties
Only need minimal medical training
60 seconds assessment based on:
- Ventilation
- Perfusion & Radial pulse
- Neurological Status
Only treat life threatening problems on site
Triage Classification 6
Walking Wounded

Directed to holding area

Subsequent assessment for hidden
injuries

Triage Classification 7
Non-Ambulant Casualties

Category Respiration Radial Pulse LOC

Critical/ > 30/min Absent Unconscious
Immediate or AMS

Delayed < 30/min Present Normal

Dead / Absent Absent Unconscious

Non-salvageable

Triage Classification 8
Problem

Possibility of missing out critical conditions

e.g. early shock, spinal shock.

Triage Classification 9
Triage Acuity System in Hospital
3 most common systems (Thomas & Dains, 1992)
Type 1- Traffic Director (Non nurse)
Type 2 - Spot Check/Advanced Triage
(Usually 3 categories system)
Type 3 - Comprehensive Triage
(4 or 5 categories system with written protocols)

Triage Classification 10
Patient Acuity Category (PAC)
Scale
PAC Scale 1
state of cardiovascular collapse or in
imminent danger of collapse
requires immediate treatment and maximum
allocation of staff and equipment

Triage Classification 11
Patient Acuity Category (PAC)
Scale
PAC Scale 2
Patient:
is ill, non-ambulant and is severe distress.
is not in danger of imminent collapse.
requires early attention.
requires a trolley.

Triage Classification 12
Patient Acuity Category (PAC)
Scale
PAC Scale 3
Patient:
has acute symptoms
is ambulant or requires a wheelchair

Triage Classification 13
Patient Acuity Category (PAC)
Scale
PAC Scale 4
Patient:
can be managed in a primary health care
facility.
has an old injury/chronic condition/a non-
emergency condition.

Triage Classification 14
SUMMARY
Acuity refers to how ill an illness or injury can
be.
Protocols are necessary for assignment of acuity
categories.
Category maybe changed based upon change in
patients condition.
UPTRIAGE when in doubt. Avoid downtriage.
