 Ns. Diagnose : Colorectal Cancer
 Goal : reduce pain, increase of appetite, and remove the tumor
from inside the colon with chemotherapy
 Objective : abdominal pain, cramping, constipation, loss of
appetite, weight loss, blackish stool, fatigue
 Ns. Intervention : Pain management
1. Independen :
a. Help patients to measure the scale of pain
R : Scale of pain can determine the level of pain experienced by
the patients whether the pain is mild, moderate or severe
b. Encourage patients to do deep breathing relaxation
techniques
R : Breath relaxation in helping patients distract the pain
c. Encourage nutritional intake
R : Adequate intake of nutrients is necessary to meet energy
needs and build energy reserves for activity
2. Collaborative
a. Collaboration with doctors to administer analgesic
drugs
R : Analgesic drugs can reduce the pain experienced by
patients
