Definition :

upon a health history and head to toe or general systemic
examination.
• It provides foundation for the nursing care plan.
Purpose :
• To obtain physical data about the client function.
• To establish baseline details about pts condition.
• To confirm data obtained in patients health history.
• To establish diagnosis and plan the patient care.
• To evaluate physiological outcomes of health care.
• To establish therapeutic relationship with the patient.
• To understand any changes in conditions of disease.
• To contribute to the medical research.
