Registration Requirements
A Memorandum concerning the A&E Test
Registration will be issued this April 2017.
1. Original and Photocopy of Certification of
Completion of ALS Program (DepEd or Non-
DepEd ALS provider)
2. Original and Photocopy of Certificate of
Rating of Portfolio
3. Original and Photocopy of Birth Certificate
(NSO)

Registration Requirements
4. If Birth Certificate (NSO) is NOT available, any
of the following documents can be presented:
a. Baptismal Certificate
b. Voter’s ID (with picture and signature)
c. Passport
d. Driver’s License
e. Any legal document bearing the applicant’s
picture, name and signature
5. Two (2) 1x1 identical ID Photo (white background
w/ name tag)
