ROLE TRANSITION

DEFINITION:

Implies the change in one’s role


requirement,expactions and work
responsibilities.It also requires an internal change
in the way one thinks about or views the new
role.

 Developmental Transition:

 Includes birth, adolescence, menopause, aging,


and death.

Health And Illness Transitions:

 Include recovery process, hospital discharge, and


diagnosis of chronic illness

Situational Transitions:

 Involve the addition or subtractions of persons in a


preexisting constellations of role and complements. (e.g
loss of family member through death) 

Organizational Transitions:

 Changing environmental conditions that affect the lives


of clients, as well as workers within them .

Nurses prepare to enter the profession from
student to registered nurse (RN) .
They move not only from one role to another,
but also from the school or university setting
to the workplace.
FACTORS INFLUENCING ROLE TRANSITION

1. Not feeling confident and competent


2. Making mistakes because of increased
workload and responsibilities
3. Encountering new situations, surroundings
and procedures
4. Inconsistent preceptors
5. Getting to know the staff
REALITY SHOCK
 ‘‘The reactions of new workers when they find
themselves in a work situation for which they
have spent several years preparing, for which
they thought they were going to be prepared,
and then suddenly find they are not”

CAUSES OF REALITY SHOCK

Feel unsure without the security of the


instructor
Education program did not adequately prepare
Do not have time & adequate knowledge
Fear, lack of confidence, total responsibility
Unrealistic expectations of self and co-workers.
PHASES OF REALITY SHOCK
This is also known as Kramer’s model of reality
shock/ Kramer’s theory of new nurse socialization.

There are four phases of reality shock :


Honeymoon phase.
Shock or Rejection phase.
Recovery phase.
Resolution phase.

Honeymoon phase
  Period of excitement and seeing the world through ‘rose colored glasses’
 Everything in the work world is just as the new graduate
imagined.
 Most often occurs during the orientation period.
 The new nurse is in orientation with former school friends
or other new graduates who often share similarities.

 Develop initial bonds of trust and respect .

Shock phase:
Negative feelings towards their new profession arises.
During this phase the nurse receives daily assignments and
begins the tasks.
The novice nurse may be frightened or react by forming a
hard cold shell around himself or herself.
The nurse comes into contact with the followings.

Lack of security of an expert available as a resource

 Finding a mentor for guidance,
 Taking care of yourself physically and emotionally,
 Developing a support network.

Recovery Phase :
 Able to look at all sides and see the realities with a
more open perspective.
The novice nurse begins to understand the new
culture to a certain degree. There is less tension and
anxiety, and healing begins.

Resolution Phase :
 Usually take around a year. Person can see the role in
perspective and fully contributes to the profession.
The resolution phase is the result of the shock phase and the
novice nurse's ability to adjust to the new environment. If the
nurse is able to positively work through the rejection phase, he
or she grows more fully as a person and a professional nurse
phase.

Tips:
 Continually help them retain the positive aspects of their job.
TRANSITION FROM NOVICE TO EXPERT

There are five stages which novice nurse


proceed to be more clinically competent:
Stage 1 NOVICE: The nurse has few experience
with clinical expectation and skills are learned
by rote. This stage usually occur while
completing the nursing educational
requirement.

Stage2 Advanced Beginner: Exemplifies
advanced beginners who are able to perform
adequately and make some judgment calls
based on experience most novice nurse enter
the work force during this stage .

Stage3 Competent: Includes competent nurse who are


able to foresee long range goal and are mastering
skills.

Stage4 Proficient: Includes proficient nurse who view


whole situation rather than part and are able to
develop solution.
Stage5 Expert: Includes expert nurse with whom
intuition and decision making are instantaneous .
SPECIAL NEED OF NOVICE NURSE

1- Interpersonal skill.
2- Clinical skills .
3- Organizational skills.
4- Delegation skills .
5- Priority setting skills.
INTERPERSONAL SKILLS :

 Most physician administrators and the head nurse


expect the novice nurse to immediately develop
interpersonal skills that they take for granted.

 They had time to get to know the members of the


health care and felt comfortable interacting with
them.
CLINICAL SKILLS

• The novice nurse has basic knowledge of how


to perform nursing skills

• Practice increases effectiveness ,efficiency and


correctness of performing skills.
ORGANIZATIONAL SKILLS

 The novice nurse,may lack organizational


skills.

 Someone is usually with student to offer


suggestions on how to organize their time.
DELEGATION SKILLS

 Most student nurse have limited exposure to


delegation uncertainty or feeling
uncomfortable with delegation may be result
of the personnel to whom one is delegation.
PRIORITY SETTING SKILLS
 Priority setting is a skills that all nursing
student must demonstrate .
 The difference between nursing school and
the real world is that serious consequence
occur if prioritizing is not done effectively
STRATAGIES TO EASE TRANSITION
1-BICULTURALISM:
Biculturalism is the joining of two
contradictory value system.

2- ROLE MODEL AND MENTOR:


A staff nurse can be role model for a novice
nurse but have no interaction with the novice.

Mentor help novice nurse set and reach


realistic goal by reinforcing and recommending.
STRATEGIES TO EASE TRANSITION
3-PRECEPTORSHIPS:
Preceptor program often are
Incorporated during the senior nursing student
final practicum.
4- SELF MENTORING:
 Novice nurse must be willing to learn
appropriate reference .
 Develop problem solving skills and ask question .

5-SELF-CONFEDENCE & SELF ESTEEM.

 Self esteem= self confidence + self respect.

 Individuals with high self esteem can critically


solve problem & tackle obstacles .
KEYS TO SURVIVAL DURING TRANSITION

1 . Support yours co-workers, be good listener

2. Treat yourself , do something nice every week.

3. Avoid people who irritate or hassle you, pessimistic


people can bring you down .
4. Say no and don’t feel guilty .

5. Remember nursing is a noble profession .


RULES OF THE ROAD
1-STOP:
Take care of your self.Take time to plan your
transition.

2-DETOUR:
You will make mistakes. Recogonize
them,learn from them,and put them there is
always detour ahead.

3-CURVE AHEAD:
Get your personal life in order.Anticipate
changes in your schedule.Be adoptable, because
the transition process is not predictable.
4-YIELD:
You don’t always have to be right.Consider
alternatives,and make compromises within your
value system.

Maintain a positive attitude. As you gain
experience,you will become better organized and
begin really to enjoy nursing.

6-EXIT:
Pay attention to your road signs;do not take an
exit you do not really want .Before you exit your
job,critically evaluate the job satisfaction.

7-SLOW TRAFFIC,KEEP RIGHT:
You may be more comfortable in the slower traffic
lane with respect to your carrer direction.Take all the
time you need;it is ok for each person to travel at a
different speed . Do not get run over in the fast lane.
8-SCHOOL ZONE:
Plan for continuing education, whether it is an
advanced degree program or one to maintain your
clinical skills or license.
