PERINEAL CARE

Purpose
To promote cleanliness by removing transient
microorganisms, secretions, and excretions in the
perineal area.
Nursing Considerations:
1. Perineal care is also referred to as peri-care.
2. Perineal care is a procedure that the most clients find
embarrassing. Encourage the client to perform this
procedure on their own with minimal assistance as
mush as possible.
Equipment:
Bath basin filled with warm water
Bath towel
Bath blanket
 wash cloth
Cotton balls or gauze
Bed pan
Wash cloth
Disposable gloves
Nursing Action
1. Fold the top linens at the foot of the bed. Fold the
client’s gown to expose the genitalia.

2. Secure a bath towel under the client’s hops. Bath


towel protects the bed from being soiled besides
using it for drying the lower end of the genitalia and
the upper end of the rectal area.

3. Position the client in dorsal recumbent position.


4. Drape the client with bath blanket and fold the lower
middle part of the blanket to expose the genital area. This
promotes warmth and prevents unnecessary exposure.

5. Do gloving and inspect the area for inflammation,


swelling, secretions, and presence of odors. Pay particular
attention to the labia in female and scrotal folds in males.

6. Clean the genital area.

7. Rinse and dry the perineum thoroughly.


FEMALE
A. Clean the labia majora. Then spread labia to wash
folds between labia majora and minora using
separate corners of the washcloth for each stroke.

B. If the client is menstruating or indwelling catheter is


inserted, clean the labia using separate cotton ball or
gauze for its stroke.
 Secretions that accumulate from the labia minora
supports bacterial growth.
 Wiping is done from the area of the least contamination to
the area of greater contamination.

 Or, you may wash the perineum by placing the client on a


bedpan or pouring warm water over the area.
 Rinse and dry the perineum thoroughly.
MALE
1. Clean and dry the penis. If the client is
uncircumcised, retract the prepuce exposing the
glans penis. Then replace it after cleaning. This is
necessary to remove the smegma that collects to the
foreskin.

2. Wash and dry the scrotum.


7. Clean and dry the buttocks.

8. Discard or dispose the equipment to appropriate


receptacle.

CHARTING:
Document the performance of the procedure and
assessment on the perineal area.
