Instrument assisted soft tissue mobilization (IASTM) is a technique that uses tools to break down scar tissue and adhesions in soft tissues like fascia and muscles. It separates collagen cross-links and stretches tissues to increase range of motion. Certification in IASTM, such as Graston technique, requires hands-on training over 1-2 days. As a physical therapist assistant, certification in IASTM would allow me to incorporate additional soft tissue interventions to improve patients' pain and function.

GRASTON ( IASTM)

BY: ALEX GARCIA

WHAT IS IASTM?? OVERVIEW

Instrument assisted soft tissue mobilization is a skilled myofascial intervention that

is used for soft tissue treatment. It utilizes a stainless steel or sometimes other
material tool that is then applied to the skin with the purpose of treating soft tissue
dysfunctions. The goal when using this tool or treatment intervention is to separate
and breakdown collagen cross Links , adhesions. It Stretches and facilitates
realignment of muscle fibers, fascia and connective tissue .Also it can increase
blood flow to the area, increase histamine release and facilitates muscle relaxation.
It is based on the principles of James Cyriax cross friction massage. It can be an
alternative to manual therapy or it can be utilized before or after Manual therapy.
CONTINUED OVERVIEW

• Between the skin, muscles and bone we have layers of fibrous connective tissue called fascia, Which is
primarily made of collagen fibers. The purpose is to stabilize and allow flexibility of those tissues such as
ligaments, skin muscles and other internal Compositions.
• In healthy tissue muscles should be to slide without restriction, when there is an injury present in the soft
tissue fibroblast release more collagen Causing an increase in the density of the fascia which ultimately lead
to adhesions.. This scar tissue limits the function at a range of motion of the muscles and joints in the
affected region.
• IASTM assists in releasing and loosening up Fascial adhesions Utilizing these instruments and various
techniques by pulling, stretching and releasing fascia tissues, promote increase Blood flow to capillary
beds . Assist in stimulation Neurological pathways and reflexes which Affect muscle tone and mobility.
INSTRUMENT ASSISTED SOFT TISSUE
MOBILIZATION ( IASTM)
Types
• Graston
• Hawkgrips
• Rocktape
• ASTYM
• FAKTR
• Adhesion breakers
• each company has its own treatment approach with different instrument designs
INSTRUMENT ASSISTED SOFT TISSUE
MOBILIZATION TECHNIQUES
• Sweeping
• Fanning
• Brushing
• Strumming
• Framing
• These are all based on the body region as well as the nature of the soft tissue restriction
and the outcome that is desired through the technique.
BENEFITS OF IASTM

• Separates and breaks down collagen cross-links

• Stretches connective tissue and muscle fibers
• Increases skin temperature
• Facilitates muscle relaxation
• Alter spinal reflex activity
• Increases blood flow to the area
• Increases histamine response
PRECAUTIONS

• Cancer
• Burns
• Wounds
• kidney dysfunction
• Pregnancy
• anticoagulant meds
• RA
CONTRAINDICATIONS

• Any open wounds or sutures

• Thrombophlebitis( blood clot in a vein that causes inflammation and paIn >>DVT)
• Uncontrolled hypertension
• Unhealed fractures
• Hematoma/myositis ossificans
• Osteomyelitis
IMPORTANT INFO TO BE CONSCIOUS OF WITH
IASTM
• Typically you want to have the patient warm up the body area first In order to increase
blood flow prior to performing IASTM.
• Patient education in regards to what they might feel or note on their skin post treatment
such as minimal discomfort, redness, possible bruising.
• After performing IASTM it is ideal to instruct patient through the new range of motion
WHAT SPECIAL REQUIREMENTS ARE NEEDED?

• Estimated Cost: roughly $700

• Pre-requires: not specific, licensed physician
• A clinician must obtain a GTS Certification (Graston Technique Specialist Certification) in order to
be documented as a graston intervention when treating patients as a clinician
HOW LONG DOES IT TAKE TO GET CERTIFIED?

• How long does it take to get certified or complete this pathway: per Graston technique
website it is a hands on , lab setting , with power point presentations and
essential training materials with a fee of 650-700$, The length of time varies by
institution. Graston technique website offers a 12 hour , In person group
training held over two consecutive days at a regional location. Or a hybrid
training which consist of a 3 hour self paced Pre-course followed by a 9 hour,
one day in person group training held at a regional location.
• Self paced training: presented in video format is another option. This self-paced online
training can be completed Anywhere when you have time to train.

WHO CAN CERTIFY IN GRASTON?

• Certified Athletic Trainers

• Acupuncturists
• Certified Hand Therapists
• Doctors of Chiropractic
• Occupational Therapists and COTAs
• Physical Therapists and PTAs
• U.S. Trained Massage Therapists
• Canadian Registered Massage Therapists
HOW OFTEN DOES THE CERTIFICATION NEED TO
BE RENEWED?
• As a licensed PTA in the state of Illinois must complete 20 CE hours each license cycle.
( renewal in odd years.)
• A Graston technique specialist ( GTS) must be renewed every .
PT/ PTA CERTIFIED IN IASTM

• Name: Emily Birney

• Job title: PTA , Graston certified ( GTS)
• Their practice location: team rehabilitation physical therapy outpatient clinic ( bucktown)
HOW WOULD THIS BE BENEFICIAL TO ME AS A PTA IN
ORDER TO ADD TO THE SETTING I HOPE TO WORK IN?

• I would benefit from this certification as a future clinician by being able to implement
Graston technique and tools to my treatment sessions to help my patient decrease tightness
in the muscular tissue by stretching out restricted fascia, Connective tissue in order to
reduce pain and tightness and increase circulation to the area. Which will then improve the
patient’s symptoms which will ultimately Improve their functional mobility and allowed
them to carry their activities of daily living.. Having this certification and knowledge will
benefit me in the outpatient setting which is where I plan to work in .
• Incorporate an extra intervention or tool to my knowledge and save my hands from constant
manual therapy.
