PERMIT TO WORK TRAINING
PERMIT TO WORK TRAINING SCOPE

• This Standard applies to all company sites and those of subsidiary


companies of Ltd.

• Non-production work e.g. maintenance, repair, inspection, testing,


alteration, construction, dismantling, adaptation, modification of
equipment or processes, cleaning etc.

• Temporary or permanent changes.

• Jobs where two or more individuals or groups need to co-ordinate


activities to complete the job safely

PERMIT TO WORK TRAINING

• A form, which sets out, agreed work to be done on identified equipment, or in an

identified area, and the precautions to be taken. It is clear record that all

foreseeable hazard have been mechanism of equipment/system between the

owning group and another group responsible for executing this work. There shall

be at least three different types of PTW forms:

• Hot Work Permit, Work at Height Permit & Civil Work

• Cold Work, Confined Space & LOTO Permit


WHAT IS A WORK PERMIT

• Work Permit is a documented format that

authorizes certain people to carry out specific

work within a specified time frame / area. It sets

out the precautions required to complete the work

safely, based on a risk assessment.


FEATURES OF PERMIT-TO-WORK SYSTEMS

• A Standard Operating Procedure, job or method statement and information

• The equipment and tools to be used

• A hazard and risk assessment

• Required precautions, mitigating actions and control measures

• All PPE requirements

• The permit issue, extension and withdrawal details

• The names and signatures of those who are responsible for the job

• A Tool Box Talk signed by all workmen


FEATURES OF PERMIT-TO-WORK SYSTEMS

• Safe system of work - Proper planning - manage the risks for wide range of
activities -- Prevent accidents and conduct work in a safe and responsible
manner

• Written document of agreement signed & Understood

• Authorizes certain people Issuer / Executor / Field operator / AGT / Isolator --

simultaneous activity

• Joint Risk assessment - to identify potential hazards – Associated Risks, at a

WHEN ARE PTW SYSTEMS APPLICABLE

• Works which may adversely affect the safety of personnel, the environment

or the plant

• High Risk / Non-routine activities which will require some form of JSA / RA

prior to work commencing

• It is not intended that PTW procedures be applied to all activities as

experience has shown that their overall effectiveness may be weakened


WHEN ARE PTW SYSTEMS APPLICABLE

• Hot work in explosive environment work of any type where

• 1. Work which may generate sparks or other sources of ignition;

• 2.Heat is used or generated (eg by welding, flame cutting, grinding etc);

• Work on high voltage electrical equipment; Electrical troubleshooting or

repair on live circuits

• Work involving the use of hazardous / dangerous substances, including

radioactive materials and explosives


WHEN ARE PTW SYSTEMS APPLICABLE

• Pressure testing;

• Complex machinery

• Evacuation near buried services /

• Escape or rescue systems;

• Temporary equipment, e.g. generators, welding equipment etc;

• Critical Lifting, Use of mobile cranes

• Bypassing or removing / altering Safety devices or equipment


WHEN ARE PTW SYSTEMS APPLICABLE

• Work at height; Elevated works

• Operational pipelines

• Other applications

• Insulation or catalysts handling

• Product conversion of stationary or mobile or portable vessels and containers

• Exposure to moving / rotating machinery

• In proximity of vents, liquid of gas

• On process lines with gas release

• Confined spaces

Entry and work

Potential oxygen deficiency or enrichment Potential flammable/explosive

atmosphere Potential high temperature/pressure Potential hazardous

chemicals, e.g.: toxic substances Confined space entry, e.g.: tanks, cold box, pit,

normally closed vessels


WHEN ARE PTW SYSTEMS APPLICABLE

• Excavating

• The safety representative must verify that all underground utilities have been

located and staked out. Provisions must be made to barricade the open

trench, prevent collapse of trench walls, provide employees with a means of

exit, Reroute ground water, all other requirements of excavating If soil is

contaminated, safety and health plan.


WHEN ARE PTW SYSTEMS APPLICABLE

• Exposure to Traffic / Vehicles / Combustion Engines

• Internal combustion engines or vehicles with internal combustion engines inside

company buildings. produce poisonous gases such as carbon monoxide, and oxides of

nitrogen. Use of company owned equipment, such as personnel lifts, fork trucks,

vehicles, etc., by employees of a contractor Prevent equipment emissions from

contaminating air inside buildings and confined spaces.

• Movement of large pieces of equipment, Especially while other personnel are working

nearby, can result in employee injury from falling material, or being struck by equipment.

• The issuer shall along with the Acceptor of the permit shall visit the job spot and

jointly assess possible hazards,

• The issuer shall complete the PTW, specifying the task to be done possible

hazards, PPE to be used and attach separate Method Statements it required.

This shall be done in consultation with engineering manager officer ( for

engineering jobs) and the contractor supervisors, process) and get these on

the permit as appropriate.

• After filling the Precautions Checklist the permit then shall be approved/

authorised by the issuer in case of engineering jobs the permit shall be

authorised jointly by the issuer the designated engineering officer manager.

• The hand over form issuer/Engineering officer (or manager) to Acceptor must

include a visit to the job location. The Acceptor shall signs the permit. ( both

original and the cop in the permit book). The first copy shall be handed over

to the Acceptor. However acceptor’s signature area mandated only in case

where the job is contracted.

• The period of validity for a PTW in defined areas within a site shall be the

estimated time for the completion of the job, but no more than 8 hours or the

period during which the issuer/ Engg officer is re-authorised by the respective

Reliever(s) after re assessment of the job location.


PERMIT TO WORK TRAINING OPERATION

• An Acceptor needing to continue with the job into the next shift period must be
asked to contact the issuing plant in the next shift and ensure the validity is
extended provided that no change has taken place in the conditions stipulated in
the permit.

• The permit issued on a particular day may be extended. If required only for the
shifts on that particular day. For work extending beyond the day and to continue
on the next day(s) a fresh permit shall be issued as per the requirements stated in
examining the job location and reassessment of hazards as well as identifying the
name of persons who will carry out the job.

• If the team/abandoned then the Acceptor will sign off the permit as “ Work
Suspended” the PTW and the permit to the issuing plant. The issuing plant shall
formally close such permits after examining the job location ( A new permit will be
required for restarting the job)

• The acceptor (i.e. the contractor supervisor or own employees on

the job) must detail on the permit to Work and describe to the

issuer to precise state of the equipment when the task is finished,

and confirm the area has been left clean and tidy. He will confirm

this by signing the permit.
