DENGUE
INTRODUCITON AND EPIDEMIOLOGY
• Dengue fever is an acute febrile illness characterized by fever, myalgia, arthralgi and rash. • Severe dengue infection is characterized by abnormalities in hemostasis and by leakage of plasma from the capillaries which may lead to shock and death. EPIDEMIOLOGY: • The globlal prevalance is increased • Endemic in more than 100 countries in Africa, America and the Eastern Mediterranean, Southeast Asia and Western Pacific. • Studies estimated more than 90 million annual symptomatic cases of dengue across the world. • Attack rates approx. 40 -50% • An estimated 5,00,000 cases with severe dengue fever need hospitalization • 2.5% dies due to dengue fever. • Without proper diagnosis and treatment the mortality rate can increase upto 20% due to dengue hemorrhagic fever. • Pre-dominant vector is the urban species Ades aegypti • A rapid rise in urban population increases number of people into contact especially due to breeding sites. VIRUS: • Arbovirus • Family – Flaviviridae • RNA type of virus • Dengue fever is caused by four serotypes of Arbovirus. • Approximately 50nm in diameter with approximately 1100 nucleotides. • The envelop protein bears the epitopes that are unique to each serotypes. • Antibodies to these epitopes neutralizes the virus by interfering with entry of virus into the cells. • Four well defined serotypes are: DENV-1, DENV-2, DENV-3, DENV- 4 TRANSMISSION: • Transmitted through bites of infected female Ades mosquitoes. • They acquire virus from the infected person through feeding. • Incubation period is 3-7 days in mosquitoes and they are suceptible to infect healthy individuals • Infected females also transmitt the virus through transovarial into their offsprings. • Humans are the main amplifying hosts. • The virus circulates in the blood for 2-7 days as approximately as same time as fever.
