PROBLEM STATEMENT

A 62 year old male presents with a non-


malignant Prostate enlargement after a
digitorectal examination. He is however worried
about his inability to maintain an erection and
pass urine effectively.

MECHANISM OF ERECTION
1. Stimulus
a. Tactile stimulation of the glans penis
b. Psychic stimulation from the brain

2. Parasympathetic impulses to the penis through the pelvic splanchnic nerves

3. Nitric oxide, Vasoactive Intestinal Peptide and Acetylcholine release by


parasympathetic nerve endings

4. Activation of Guanylyl Cyclase by nitric oxide

5. Dilation of arteries and relaxation of smooth muscles in erectile tissue

PSYCHOLOGICAL EFFECTS OF ERECTILE
DYSFUNCTION
Effects on the affected individual;
● Feelings of emasculation and humiliation, decrease in self confidence and self
worth
● Can also lead to depression, isolation, loneliness and decrease in subjective
well being

Effects on the sexual partner of the affected individual


● Frustration
● Rejection
● Feeling of unattractiveness towards sexual partner

CORRELATION TO THE SCRNARIO

Due to Mr. Gyasi's enlarged prostate compressing the prostatic part of


his urethra, he has to contract his lower abdominal muscles more to
push the bladder down and allow urine out of the urethra.
His erectile dysfunction can be attributed to his enlarged prostate
compressing the prostatic plexus which give off cavernous nerves
needed for erection of the penis.
