- Bei der Osmotherapie werden dem Körper mittels intravenöser Infusionen osmotisch wirksame Substanzen zugeführt. In der Regel handelt es sich um hochkonzentrierte Lösungen von Zuckeralkoholen (z. B. Mannit, Sorbit). Durch ihren hohen osmotischen Druck sollen sie Flüssigkeiten binden, die sich im Gewebe angesammelt haben, so dass diese über die Nieren ausgeschieden werden können. Durch den Wasserentzug können Ödeme behandelt werden. Die früher weiter gefassten Anwendungsgebiete der Osmotherapie (Lungenödem, Bronchitis oder frische seröse Pleuritis) haben durch die entstehende Rechtsherzbelastung eine Indikationseinschränkung erfahren. Die Osmotherapie wird vor allem zur Behandlung des krisenhaften Anstiegs des Hirndrucks (intrakranieller Druck, ICP) angewendet. Bei dem Schädel-Hirn-Trauma ist Mannit die erste Wahl und scheint die Letalität bei Anwendung einer mittels Druckmessung gesteuerten Indikation zu senken. Von der Osmotherapie sollte bei der zerebralen Massenblutung Abstand genommen werden. Weitere Anwendungsgebiete sind die Senkung eines akut erhöhten Augeninnendrucks (Glaukom) und die Aufrechterhaltung des Harnflusses bei einem drohenden Nierenversagen. Die gezielte Einleitung einer osmotischen Diurese kann in der Diagnostik einer unzureichenden Harnbildung (Oligurie, Anurie) durchgeführt werden. (de)
- Osmotherapy is the use of osmotically active substances to reduce the volume of intracranial contents. Osmotherapy serves as the primary medical treatment for cerebral edema. The primary purpose of osmotherapy is to improve elasticity and decrease intracranial volume by removing free water, accumulated as a result of cerebral edema, from brain's extracellular and intracellular space into by creating an osmotic gradient between the blood and brain. Normal serum osmolality ranges from 280 to 290 mOsm/kg and serum osmolality to cause water removal from brain without much side effects ranges from 300 to 320 mOsm/kg. Usually, 90 mL of space is created in the intracranial vault by 1.6% reduction in brain water content. Osmotherapy has cerebral dehydrating effects. The main goal of osmotherapy is to decrease intracranial pressure (ICP) by shifting excess fluid from brain. This is accomplished by intravenous administration of osmotic agents which increase serum osmolality in order to shift excess fluid from intracellular or extracellular space of the brain to intravascular compartment. The resulting brain shrinkage effectively reduces intracranial volume and decreases ICP. (en)
