- Als Mikrolithiasis testis oder testikuläre Mikrolithiasis bezeichnet man das Auftreten von multiplen ca. 1–3 mm großen Verkalkungen im Hoden, welche über das gesamte Gewebe des Hodens verstreut sind. Im Ultraschallbefund ergibt sich ein Bild von vielen kleinen hellen Punkten, das als Sternenhimmelphänomen bezeichnet wird. Der Befund findet sich bei bis zu 5 % junger Männer als Zufallsbefund und hat keinen eigenen Krankheitswert. Die Bedeutung des Befundes als Risikofaktor für Keimzelltumore beim Mann wird kontrovers diskutiert. (de)
- Testicular microlithiasis is an unusual condition diagnosed on testicular ultrasound. It is believed to be found in 0.1 - 0.6% of males globally, with frequency varying based on geographic location and is more often found in individuals with subfertility. It is a often an asymptomatic, non-progressive disease; though in a very small number of cases it may also cause bouts of extreme chronic fatigue, hormone imbalance and pain, with can be severe and accompanied by swelling around the testicular region (dependent on the size and location of the calcification). In an extremely rare select few cases, individuals with microlithiasis have also been known to experience calcification of the prostate, which can lead to the passing of stones. These rare cases can lead to secondary infections if not treated with care, due to the resulting damaged tissue. It is important to note however that these symptoms are rarely seen in the majority of people who have been diagnosed. Testicular microlithiasis is not associated with risk of testicular cancer in asymptomatic individuals. However, a large meta-analysis has shown that in individuals with associated risk factors for testicular germ cell tumor, the increase in risk of concurrent diagnosis of testicular germ cell tumor, or testicular carcinoma-in-situ upon biopsy is approximately eight to ten-fold. There is extensive controversy over whether testicular microlithiasis in individuals with testicular germ cell tumor, or risk factors for such, should undergo testicular biopsy to exclude the presence of testicular carcinoma-in-situ, also known as intratubular germ cell neoplasia of unclassified type. Additionally, whether the presence of testicular microlithiasis should influence decision for adjuvant chemotherapy or surveillance in individuals with testicular germ cell tumor remains unclear. A recent review in Nature Reviews Urology has comprehensively evaluated these topics. It is important to note however as with any condition that symptoms can vary from individual to individual. (en)
- Тестикулярный микролитиаз (Киста яичка) – одна из болезней в большом специализированном разделе медицины. Парными мужскими половыми железами тестикулами (яичками) и парными секреторными органами эпидидимисами (придатками яичка) вырабатываются сперматозоиды, а также – некоторая часть гормон тестостерон. В верхних фрагментах желез, в части их придатков, а также по направлению вдоль семенного канатика создается благополучная среда для кисты яичка – своеобразной полости с фиброзной оболочкой, с содержимым жидкого плана. Кисты яичка – это доброкачественные опухолевидные образования. Это одно из самых распространенных заболеваний мошонки. Они обнаруживаются приблизительно у трети обследованных пациентов. Для обнаружения их достаточно ультразвукового исследования мошонки. При кистах придатков яичек и семенных канатиков выраженной клинической картины, как и признаков внешнего характера, обычно нет. Иногда заметно деформируется мошонка, есть ряд других симптомов. Пациент в большинстве случаев обращается к урологу-андрологу при болевых синдромах. Сами кисты незаслуженно не считаются поводом обращения к специалисту. Но такое отношение понятно, ведь эти образования не обладают никакой симптоматикой и обнаруживаются во время диспансеризации, благодаря осмотру уролога. (ru)
- A microlitíase testicular é uma doença caracterizada pela presença de microcalcificações no interior dos túbulos seminíferos presentes no testículo. Sua patogênese é controversa e sua relevância clínica é incerta. Esta doença geralmente é descoberta incidentalmente durante a ultrassonografia do escroto. Pode ser encontrada em indivíduos normais ou em associação a uma grande variedade patologias testiculares benignas e malignas, principalmente dos tumores testiculares. Dentre as patologias benignas, é descrita sua associação com a síndrome de Klinefelter, criptorquidismo, subfertilidade/infertilidade, infecções (tuberculose, filariose, etc), trauma, varicocele, cisto de epidídimo e neoplasias benignas, incluindo tumor carcinóide primário do testículo, teratoma maduro, tumor adenomatóide e tumor do cordão sexual-estroma. Dentre as neoplasias malignas, as microcalcificações foram observadas em casos de seminoma, teratoma maligno, tumor carcinóide metastático e carcinoma de células embrionárias. Em 1982, Ikinger e col., estudando 43 tumores testiculares demonstrou uma freqüência de 60% de microcalcificações nestes testículos e sugeriu que a microlitíase seria um sinal específico e precoce das neoplasias testiculares. Outros trabalhos, entretanto, defendem que esta associação seria casual, primeiro, pela ocorrência de microcalcificações nos testículos de pacientes com criptorquidismo e subfertilidade/infertilidade, fatores de risco conhecidos para o desenvolvimento destas neoplasias. E, segundo, pela presença de microcalcificações no testículo contralateral sem tumor. Estudos científicos não comprovam se a microlitíase pode ser considerada a causa de outras patologias. Entretanto, há evidências que justificam o acompanhamento ultrassonográfico dos pacientes portadores de microlitíase com a finalidade de diagnosticar precocemente os tumores testiculares. (pt)
