Dear Sir/Maam,

I write in connection with a case for violation of Batas Pambansa Blg. 22, or the
Bouncing Checks Law, which I have filed and which is now pending before the
Municipal Circuit Trial Court (MCTC) at Gerona, Tarlac, docketed as Crim Case No.
XXXXXXX and captioned as XXXXXXXX. I am the payee of the said check which was
issued by the defendant XXXXXXX.

My original copy of the check has been lost. I therefore request that a copy of
said check or a true and authenticated copy thereof be furnished me. The said check
has the following details:

RCBC Savings Bank Check No. XXXXXXXX

Account No. XXXXXXX

Date of issue of the check: XXXXXXXX

I previously attempted to deposit the said check with the Planters Development
Bank Mc Arthur Highway, but the same check was dishonored and returned on May
11, 2015 by reason of ACCOUNT CLOSED and A/C as stamped on the check.

I make this request pursuant to the evidentiary requirements of the aforecited


case that I have filed.
