REASON FOR REFERRAL:


Mildred Johnson agreed to participate in the Woodcock Johnson IV assessment to assist a
student in assessment training.

BACKGROUND INFORMATION:
Mildred Johnson is a Caucasian female, age 13 years 9 months. She stated that she is currently
enrolled at Woodbury Middle School and is in the 8th grade. She also described to me that she is
taking advanced math at her school, but does not enjoy math.

BEHAVIORAL OBSERVATIONS:
Mildred Johnson is average height and weight for her age, with a medium build. She came to
testing comfortably dressed in a t-shirt and athletic shorts. She wore her long dark brown hair
down and it appeared to be neat and clean. Mildred was very talkative and giggly throughout
the testing session. She had a very positive attitude while answering questions, however when
she was answering a difficult question she would ask me if she put down the correct answer or
told me to tell her what the correct answer was. She was very compliant, clearly attentive while
listening to directions. In addition, she was undoubtedly concentrating when answering
problems throughout testing. Mildred did not appear to be anxious when beginning testing, she
seemed very excited to participate in the examination. She remained attentive and alert
throughout the testing session, although as testing progressed it was getting harder for her to
concentrate.

Mildred Johnsons dominant language is English. She did not show any issues with visual,
auditory, or motor skills. Her moods throughout the testing session were happy and silly. This
was evident throughout the conversations I had with her during testing and the fact that she
giggled a lot as well.

Mildred Johnson explicitly stated to me her hatred for math and was reluctant to begin the
mathematical subtests in the WJ-IV. When presented a difficult question in math, she attempted
but gave up easily. When taking subtest 2 Applied Problems, she solved initial problems very
quickly and with ease but had difficulties solving the harder problems later. Throughout testing,
she would state how much she hated types of math such as subtraction and percentages. When
presented with the reading and writing subtests, Mildred was very eager to begin testing
especially the writing portions. Throughout test 4 Passage Comprehension, I observed she was
having a lot of trouble. She would state to me that she did not know the answer and then would
give up.

The WJ-IV measures three basic academic skills: reading, writing, and arithmetic.
Additionally, math calculation and writing abilities are assessed. Results are reported for both
cluster scores and subtest scores in standard scores with a mean sore of 100 and standard
deviation of 15.

SUMMARY
*An examination of Mildred Johnsons tests revealed an area of weakness being Passage
Comprehension, meaning she has difficulties understanding what she is reading. However,
Mildred Johnsons subtest standard scores revealed strengths in Sentence Writing Fluency, Math
Facts Fluency, Applied problems, Calculation, Sentence Reading Fluency, and Spelling. My
overall conclusions are that Mildred Johnson does not struggle with math and writing given that
she is above grade level for all of those subtests, except for letter-word identification where she
is at grade level. She does however struggle with reading. She scored at a 6.2 grade level for
passage comprehension and is at a 7.5 grade level for her overall reading. My recommendations
for Mildred would be to keep her in accelerated math. She clearly excels at math even though she
does not enjoy math. She made that clear when she would complain about it when taking the
mathematics subtests. Mildred does need extra help with her reading comprehension. She did not
seem confident, did not answer with ease, and gave up easily when taking the Passage
Comprehension subtest. She should get one on one help with a tutor or a teacher to help increase
her reading skills and level using Directed Reading Thinking Activity.
