Oryza B. Lajarani filed an affidavit to change the color of her 2002 Isuzu Hilander Crosswind XUV from yellow/black to maroon/black. I, ORYZA B. LAJARANI, of legal age, Filipino, married and a resident of


Enriquez Dr., Sta Catalina, Zamboanga City, Philippines, after being duly sworn
to in accordance with law, hereby depose and state:

That I am the registered owner of a certain motor vehicle, which is more


particularly described as follows:

Make : ISUZU HILANDER CROOSSWIND XUV

Plate Number : LEE-838
MV File No. : 1112-149635
Year Model : 2002
Color : YELLOW TO BLACK
Body Type : WAGON

That the original color of the said motor vehicle was “YELLOW/BLACK”;

That for reason that the paint of the aboved-described motor vehicle had faded,
and in order to give it a new look, I commissioned Boy Ramirez- a
mechanic/painter to paint it to Maroon/Black. Such that the color of the said
motor vehicle was changed from “YELLOW/BLACK” to “MAROON/BLACK”;

That the change of color is not intended for any illegal or unlawful purpose but
solely due to the foregoing reasons;

That I am executing this Affidavit to attest to the truth of the foregoing and in
support of the application for the change of color of the aboved-described motor
vehicle on its Certificate of Registration.
