AFFIDAVIT OF EXPLANATION

I, ELMA OLIVAR CALAYO, of legal age, Filipino, married and presently residing at
Brgy. Tula-Tula Norte, Magallanes, Sorsogon after having been duly sworn to in accordance with
law, depose and state that:

1. I was issued my first passport with No. AB638938007 sometime in September 21,
2012;

2. That said passport expired on September 20, 2017, without me having known the same
as it was not explained to me thoroughly;

3. Not knowing that passport had an expiration, I procrastinated and I failed to renew my
passport on the above-mentioned date;

4. Sometime in February 18, 2019, I decided to finally renew the said passport because I
need to travel to Kuwait;

5. However, a Department of Foreign Affair employee at Legazpi City informed me that


my passport renewal had already expired. Thus, I am reapplying for a new passport;

6. I executed this Affidavit to attest to the truth I mentioned herein and for whatever legal
purposes it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 21st day of February 2019 in
Bulan, Sorsogon Philippines.
