CONTRACT OF SERVICES

DESCRIPTIONS:
- LIPS Events Services will be responsible for the following on
___________________________.

(to finalized package inclusions)

- Unlimited consultations re: designs, package inclusions via telephone, email


and facebook on the day of the contract signing until the termination of the
agreement.
- Client will send inspirations as to the design.
- If no available inspirations, LIPS Events Services will send out choices for client
to choose including assistance to color/theme selection.
- Client is responsible for the suppliers’ bond required by the venue/hotel, will
shoulder the electric charges for par led lights as may be deemed necessary
and corkage for desserts for the dessert table.
- Decoration props and accessories are not allowed for take home, otherwise,
client shall shoulder refund of the props missing.

FEES & PAYMENT SCHEDULE:

- Package Price : P ____________________


- Deposit 50% : P __________ (due upon contract signing)
Balance 50% : P __________ (due on or before December 20, 2016)
Payments must be in cash or check. A deposit of 50% is required for
confirmation of Event Date. Deposit is non-refundable. A final consultation will
be scheduled about 5 days prior to event date. Balance of P____________ is
due and demandable on or before November 4, 2016.

CHANGES/CANCELLATIONS:
- Any changes made to this letter of agreement must be made in writing and
signed by all parties. You may cancel this agreement, in writing, for any
reason, however, downpayment made is forfeited.

- If the event is cancelled 5 days before the date – except for the reason of
death – there will be no refund.

- In case of non-full payment 1 day prior to the event date, this contract is
deemed terminated.

If your understanding parallels mine, please sign one copy of this letter and return it to
me along with your 50% downpayment/booking fee.
