AFFIDAVIT

I, CLIM M. FLORES, of legal age, single, Filipino and a resident of


Brgy. Sta. Cruz, Socorro, Surigao del Norte, after having sworn to in
accordance with law, hereby depose and say:

1. That I am applying for Electric Service Connection before


Siargao Electric Service Cooperative, Inc.(SIARELCO);

2. That on 2007, I and NENETH L. ARILLA in good faith mutually


agreed to become husband and wife by voluntary
cohabitation;

3. That I am still single, under Philippine Laws, as I have never


contracted any marriage with NENETH L. ARILLA or any other
person up to present;

4. That I am executing this Affidavit for the purpose of complying


with the requirements relating to my aforementioned
application;

5. That I am executing this affidavit to attest to the truth of the


above –statement for whatever legal purposes that this
affidavit may serve.

IN WITNESS WHEREOF, I have hereunto set my hands this 16th day


of April, 2018 at Surigao City.
