JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, Filomena T. Malla-ao, and Perfecto Fiaching, both of legal ages, Filipino citizens,
and residents of FD 245, Pinespark, Balili, La Trinidad, Benguet and No. 59-H, Purok Sadjo,
Loakan Proper, Baguio City, Philippines, respectively, after having been sworn to in accordance
with law depose and state THAT:

1. We personally know the late Roger Bangloy Malla-ao and his surviving spouse Susana
B. Malla-ao because he has been our friend for a very long time and town mates until his
death;

2. We also know for a fact that the late Roger Bangloy Malla-ao died on January 18, 2012
in Makati City;

3. Lately, Susana B. Malla-ao the surviving Spouse of Roger B. Malla-ao has applied for
Survivorship Pension Claim with the Social Security System (SSS) Baguio City,
Philippines;

4. Further, we know for a fact that the late Roger Bangloy Malla-ao is the one who looked
after and supported his first family Susana B. Malla-ao, the surviving spouse;

5. We freely and voluntarily execute this Affidavit solely for the interest of truth and justice
and not for any other considerations or motivated by any unlawful intentions;

6. Furthermore, we are executing this affidavit to support the claim of Susana B. Malla-ao
for survivorship benefits with the Social Security System (SSS); and

7. Finally, we are executing this affidavit for all legal intents and purposes this may serve
well;

IN WITNESS WHEREOF, we have hereunto set our hands this 26 th day of August 2021
at La Trinidad, Benguet, Philippines.
