WHAT ARE THE ADMISSION REQUIREMENTS TO BE SUBMITTED?

A. Required Credentials:
When applicant passed the PUPCET, he/she must submit an original copy and photocopies of the following documents
to the Office of Admission Services:

For Graduating Senior High School Student

 Original Senior High School Card (F138) or certified true copy with school dry seal
 Notarized certification of non-enrollment for high school/senior high school graduates who have not enrolled in
any technical/diploma/degree program immediately after graduation
 Certificate of Completion for high school/ senior high school
 Original PSA Birth Certificate
 Certification of Good Moral issued by the HS/SHS Principal/or Guidance Counselor with school dry seal
 Duly authenticated copy of the PEPT/ALS A&E Test Certificate of Rating, certified by the DepEd-NETRC -BALS
 School ID (for currently enrolled students) or any valid ID with picture (for previous SHS graduates)
 Chest X-Ray result with Film
 Medical Clearance issued by the PUP ITECH Medical Clinic
 Four (4) pcs. 2 in. x 2 in. colored picture with white background with applicant’s name on the photo
 One (1) pc. long brown envelope

In addition to the above admission requirements:
 Certification from high school/senior high school registrar with school dry seal and noted by the Principal that no
copy of F137A has been sent to other college or university
 Notarized certification of non-enrollment for high school/senior high school graduates who have not enrolled in
any technical/diploma/degree program immediately after graduation with "waiver" that if there is concealment
of previous enrollment, PUP enrollment becomes null and void
 For PEPT qualifier, a duly authenticated or verified copy of the Certificate of Rating and a Certification issued by
the Department of Education (CMO No.15, s. 2003)
 Duly authenticated copy of the PEPT/ALS A&E Test Certificate of Rating, certified by the DepEd-NETRC –BAL for
secondary level

Academic Year 2018-2019

Step 1. SUBMISSION OF ALL REQUIRED CREDENTIALS FOR ADMISSION

Proceed to RM 303 for filing the different kinds of Form and submit all credentials to RM 302.
 Only applicants with complete credentials required for admission will be accommodated and shall go through the admission
process.
 Freshman applicants are advised to come for interview in their appropriate attire and proper grooming (with collar and
sleeves, no rubber slippers, “tokong”/shorts; and no earrings for male).
 Non-appearance on the scheduled date of interview would mean automatic forfeiture of your slot.

Choice of intended course

Admission in PUP ITECH is on a FIRST-COME-FIRST-SERVED basis subject to the availability of slots.

In addition to the general criteria set by the University, admission to an academic program depends on the satisfaction of the

Proceed to the PUP ITECH Medical Clinic, Ground Floor, and RM 107 and bring the following requirements to secure a medical
clearance. Bring the following:
1. Student Admission Record Form 1 (SAR F1) Route and Approval Slip
2. Latest Chest X-ray result with film (taken within three months)
3. 1 pc (2x2) colored photo with white background and nameplate

Step 3. ISSUANCE OF REGISTRATION CERTIFICATE

Submit your accomplished SAR Form 1 to the PUP ITECH Registrar’s Office on the date to be announced later to get the printed
copy of your Registration Certificate.

NOTE: Present your Registration Certificate to your professors on the first day of classes.

Step 4. ISSUANCE OF STUDENT ID OFFICE OF THE STUDENT SERVICES (PUP MAIN CHARLIE DEL ROSARIO
