RE: Kate Murray (DOB 10 July 2005)

Dear Parents,

Kate Murray, who has been diagnosed with Spiral fracture of the right tibia will
be required to do further care and rehabilitation following discharge.

Currently, she accidentally broke her leg while doing gymnastics resulting to pain
and swelling in the right lower leg making her distressed and unable to ambulate.
X-ray has been done confirming the above diagnosis reason to prescribe Panadol
tablet as needed for pain. Crutches are already fitted and application of backslab
and bandage are also applied.

As part of her plan of care at home, please keep affected limb elevated above the
level of the heart with the use of a pillow when resting to ease swelling on the
first 48 hours. Moreover, to promote independence encourage the use of crutches
provided with assistance. She is scheduled for fiberglass casting on 03 September
2010. Post-operatively, please note and report immediately for symptoms:
swelling, tingling, burning or numbing sensation, inability to move toe and
increasing distress which she cannot be calmed. Moreover, to avoid
complications do not allow child to scratch, wet, heat or cut the cast.

During rehabilitation, physiotheraphy is recommended to increase muscle

strength, joint and balance. After the removal of cast, care is still required lasting
a month. Gymnastics, skateboarding and other impactlful activities for 4 weeks
is prohibited so as to achieve good prognosis of this case.

In case of question, please contact me.

Yours sincerely,
