AFFIDAVIT OF UNDERTAKING

I, of legal age, Saudi National and a resident of of Sapang Putol, San


Ildefonso, Bulacan, after having been duly sworn to in accordance with law,
depose and say:

1. That I am the respondent in Criminal Case No. for Malicious


Mischief and Alarms and Scandals;
2. That I posted a cash bond of _________________ with Clerk of Court,
Regional Trial Court of Makati City, for my provisional liberty;
3. That I hereby bind myself to appear and be present at all stages of
trial of the above-entitled case when duly notified, failing which,
the Court may declare me to have waived my rights to be present
during the trial and that trial in absentia may proceed.
