INTERVIEW QUESTIONS

 What is Accident?
 An unwanted, unexpected, unplanned, unforeseen event which results in any kind
of loss called accident
 What is Near Miss or Incident?
 An unwanted, unexpected, unplanned, unforeseen event that had the potential to
cause harm called near miss or incident
 Why is near miss reporting important?
 To stop or prevention from re-occurrence by introducing or by implemented new
other safe way to do work
 What to do if your colleague involve in any injury?
 Call for rescue & if possible provide him first aid according to type of injury
 What is MSDS?
 Material safety data sheet
 Name of any three hazards material?
 Toxic, Flammable, Reactive
 What are hazards?
 Something with the potential to cause harm called hazard
 What is Risk?
 An expression of possible loss in terms of severity & probability called risk
 Why do we need Risk Assessment?
 1. To identify the hazards associated with every step of job
2. To provide up-to-date safe procedure
3. To identify area of risk & improvement
4. To establish safe precautions for work
5. To close the gap between practice & procedure
 What is PPE?
 PPE is Personal Protective Equipment to save people or their body parts from any
type of body injury e.g. Safety helmet protect from head injury, Safety shoes
prevent from foot injury, Safety glass prevent against eye injury, ear plug for
hearing protection etc
 What are the main safety checks to be done prior to start work?
 Work permit, Task Risk Assessment (TRA), Job Hazards Analysis (JHA),
Emergency Response Plan (ERP),
 About LNG

 What is LNG?
 Liquid Natural Gas
 What is LPG?
 Liquid Petroleum Gas
 What is Sour Gas or Sweat Gas?
 In which H2S will be present called Sour Gas & free from H2S called sweat Gas
 What is prohibited items during entering to gas plant?
 Mobile phones, Lighters, Camera

 ABOUT H2S

 What is H2S?
 Hydrogen Sulphide is one of the most toxic gases handle in oil & gas field
 How does it smell? Can we smell it?
 Its smell is like a rotten Egg but we cant smell it during leakage because after
doing smell it we cant alive
 Say 3 properties of H2S?
 1. Highly toxic. 2. Flammable ignited at 292’C. 3. Colorless & its 1.2 times
heavier then air. 4. Soluble in water
 What is permissible exposure limit to work?
 Permissible limit is 10 PPM (parts per million) for 8 hour
 How would you protect yourself from H2S during leakage?
 At the time of emergency we will use or wear Gas Mask

 CONFIND SPACE

 What is confined space?


 Where man can’t perform his work due to lack of Oxygen & Toxic Nature.e.g
tanks, storage vessel, main holes, pipeline.
 What is acceptable limit of oxygen?
 Minimum Level is 19.5% & Maximum Level is 23.5%
 What are the Hazards in Confined space?
 Lack of Oxygen, Poor ventilation, toxic gases, noise, falling, combustibility
 What are the safety measures or precaution?
 Entry permit, sign board, Test the oxygen level, check Gas test, rescue procedure,
qualified attendant & Entrant, Safety Access
 What is pyrophoric material?
 The material if it expose in air will caught fire immediately called pyrophoric
material

 Welding & Cutting

 What hazards involved in welding & cutting?


 Electrical shock, Fire & Explosions, Burns to eye & skin due to hot surface &
molten leads, respiratory disorders, exposure to fumes
 What are the safety measures?
 Work permit, Secure cylinder in upright position, flash back arrestor should be
installed on Oxygen & Acetylene cylinder, clean the area & remove all
combustible material from area, cable management, eye protection by safety
glass, area should be covered by fire blanket & fire cylinder should be at place

 WORKING AT HEIGHT

 At which height safety harness should be wear?


 Above 6 feet height safety harness is required
 What PPE should be worn?
 Safety Harness, approved safety Helmet, shoes, goggle
 What are the main hazards?
 Falling, personal Injury to fatality

 LIFTING EQUIPMENT

 Why we need test certificate for lifting equipment & who will issue
certificate?
 To make sure that all lifting equipments are correct & safe for use & 3rd party will
issue the certificate.
 What are the safety precautions during lifting?
 Work permit, Area should be barricade with sign, only certified rigger & signal
man can perform the job, calibration & certification of crane devices, all lifting
equipment or gear should be checked before use
 SCAFFOLDING SAFETY

 What is scaffolding?
 A temporary elevated platform with its supporting components called scaffolding
 How much number of personal can climb a scaffold ladder at a time?
 Only one person can climb at scaffold ladder at a time
 What should be the minimum height of a guardrail?
 42-inch height required for guardrail
 What is the importance of toe Board?
 Toe board protects the object & tools from falling that kept on platform
 Can a person carry any tools or materials whilst climbing the ladder?
 NO

 RADIOGRAPHY

 Can other works be done at place of radiography?


 NO
 What are precautions of site?
 All area should be clear from worker, Appropriate Barricading with signboard &
flushing lights, safety person with Survey meter at safe place
 What is the color of Radiography sign?
 Yellow & Black (yellow background with Black writing or sign)

 WORKPLACE TALKS OR TOOL BOX TALK


(WPT OR TBT)

 What are workplace talks or tool box talk & what is the aim behind carrying
out WPT OR TBT at site?
 WPT or TBT is the talk, briefing or communication which we deliver on site for
workforce to create awareness among peoples from their job hazard & then told
them the preventive method or control measure.
 Who will carry out the WPT?
 Job supervisor will carry out WPT
 Name some WPT topic?
 Occupational health & environment controls, chemical controls, fire prevention
method, purpose of Sign & Barricading, uses & advantages of PPE’s (Personal
Protective Equipments), Electric Safety, fall protection etc.
 WORK PERMIT

 Name some types of work permits?


 Hot work permit, Cold work permit, Confined space permit, Equipment
movement permit, Excavation work permit, Electric work permit, Radiography
work permit etc.
 What is the function of PTW (permit to work)?
 It is the written permission from operation team for specific area & for specific
time to perform work & to ensure that hazards are identified & all safety
precautions have been taken
 When is hot work permit required?
 When any work can create sparks or ignition ( e.g. welding, cutting, grinding
etc.) to make a fire then hot work permit is required to make sure that all safety
control measures have been taken

 GRIT BLASTING OR SAND BLASTING

 What is grit or sand blasting?


 Sand or grit blasting is a process to clean the surface of pipe by blasting of sand or
grit with high pressure on the surface of pipe
 What additional PPE is used during grit blasting?
 Air supplied Hood, Ear protection by ear plug or ear muff
 What are safety checks before sand blasting?
 Work permit, JHA or TRA , certification & visual inspection of Hose, Hooper,
Grounding of compressor, automatic shut off valve with dead man switch

 FIRE FIGHTING EQUIPMENT

 Name of some fire fighting equipments?


 Fire cylinder, fire hydrant, fire sprinkle, fire truck, Deluge cylinder,
 List of some fire extinguisher?
 DCP (dry chemical powder) cylinder, CO2 cylinder, water foam cylinder
 What must be done after a fire extinguisher or breathing apparatus is used?
 Return it to store or go again for re-filling from authorized person or by
authorized filling station
 What type of fire extinguisher or cylinder can use in each type of fire?
 DCP cylinder
 COMPRESSED GAS CYLINDERS

 What are the main hazards?


 Sudden release of Gas can cause destroying everything which come in its path
 What are safety precautions during storage, handling or transporting?
 1. Store cylinder in shade area away from source of heat
2. Secure them in upright position with chain or holder
3. Move cylinder in designed trolley
4. Put close valve on empty cylinder

 EMERGENY RESPONSE

 What action must be taken if you are exposed to a gas leak?


 Stop the job, shutdown the equipment, wear gas mask, check the wind direction
by wind sock, go towards designated assembly point for head counting
 Why do we need gas monitoring?
 To check the oxygen level, H2S level. CO (carbon mono oxide) level & to check
LEL (lower explosive limit), in short we need gas monitor to make sure that is the
place is safe for work or not.
 What does a gas monitor display?
 It display LEL, H2S, CO, O2 Level

 EXCAVATION

 What are main hazards of excavation?


 The main hazard is “Cave In” (Sudden movement of separate soil into excavation
also falling or sliding of soil by side into excavation called cave in) it could
entrap, bury, injury, or immobilize person.
 What are safety measures of excavation?
 Benching, sloping, shielding, is to safety measure for cave in, before excavation
get NOC from utilities areas, overhead & underground areas, soil must be 2 feet
away from excavation, if excavation is 4 feet deep ladder or rump will provide
after every 25 feet distance or interval, daily inspection required before stating
work or after rain or wind storm

 PNEUMATIC OR HYDRO TEST

 What is pneumatic or hydro test?


 Hydro test is to inspect or to check the strength of pipeline according to
requirement by filling water & then giving the pressure by PCD(pressure
Controlling Device)
 What are the hazards?
 High pressure burst by sudden increase of pressure, loose connection of hose,
gasket can break, and the hazards effect can be from injury to death
 What are safety precautions?
 Inspection of all testing equipment before each time to start work, pressure
should gradually increased, calibrated pressure gauge should installed on line,
hose should secure with whip latches, pressure controlling device place minimum
10 feet away

 ELECTRICAL TOOLS OR EQUIPMENT

 What is the recommended current rating of power tools used in oil gas field?
 110V is recommended rating
 What is the precaution should you take before working on an electrical
equipment?
 Insulation, Electrical protective devices like fuse or breakers, guarding,
grounding, ELCB (earth leakage circuit breaker) PPE, good work practice means
LOTO(lock out Tag out)system

 SCBA / B.A TROLLY SET

 Why do we need BA (breathing apparatus), SCBA (self contained breathing


apparatus), trolley set?
 To protect or prevent from life threat like toxic gases or smoky atmosphere, also
where O2 will not enough means where will be the deficiency of O2. SCBA is
usually used for individual or short time job like inspection & also used for rescue
at the toxic or smoky atmosphere SCBA also used under water
 A face seal best suits for person having beard or without beard?
 Without beard
 A trolley set can be used for emergency escape
 No, Gas mask will used for emergency escape
Remember: Communication is the best tool for loose
prevention so share your knowledge & all incidents or accidents
which happened in past and convey the lesson learned.
