Dear Maam/Sir;
I would like to inform your good office regarding my INC Grade in Thesis 1 (1st
Semester-SY 2018-2019).
That the defense 1 has been complied on November 14, 2018 at 1:30 pm. Tuition
and other school fees been paid in full.
That the Dean of the Graduate studies Dr. Alma T. Gurea was present together
with my Adviser Dr. Roie M. UbayUbay, Dr. Nelson Caliat and Dr. Ferenal as members
of the panel. Dr. Gurea show us our passing grade after the defense.
That it was a late but approved defence schedule since it is already November 14,
2018.
That my INC grade was not been followed up right after because of the death of
my family member.
That I execute this affidavit to attest the authenticity and veracity of all the
documents submitted.
IN WITNESS WHEREOF, I have hereunto set my hand this 31st of May 2019 in
Poblacion, Alubijid, Misamis Oriental, Philippines.
