Subject: Suspension

This pertains to your absence without leave from work. According to


company record you did not report to work on ______, 20__. A review of
leave forms showed that you failed to submit a reason for your reported
absence.

You were given a written notice to explain for the said incident. You were
able to submit an explanation.

A hearing was conducted for this incident on _______. However, on the said
hearing you were not able to provide enough explanation for the reported
absence without leave.

According to company rules, your act is in violation of __________ that all


authorized absences requires a prior notice of ____ (_) days before the
intended date. If the said absence was due to an emergency, a call to the
office should be made before _____ on the first day of absence.

According to company records this was the second (2) incident in which you
violated the company's Employee Attendance Policy.

Therefore, after a careful review of your case and with enough evidence
presented, the Management hereby imposes the penalty of two (2) day
suspension effective today.
