You were given a written notice to explain for the said incident. You were
able to submit an explanation.
A hearing was conducted for this incident on _______. However, on the said
hearing you were not able to provide enough explanation for the reported
absence without leave.
According to company records this was the second (2) incident in which you
violated the company's Employee Attendance Policy.
Therefore, after a careful review of your case and with enough evidence
presented, the Management hereby imposes the penalty of two (2) day
suspension effective today.
