SUBJECT : Notice of Decision

DATE : 08 June 2023

After a thorough assessment and deliberation concerning your case of Absence without Official Leave
(AWOL) from March 28,2023 to May 2,2023, our management and our Administrative Hearing panel
members have unanimously decided to terminate your employment effective immediately.

Please note the following:

1. Prior your AWOL, our company management had accorded to you a considerate long leave of
absence (i.e., from February 23 to March 27,2023) plus a cash advance without interest.
2. Post your approved long leave of absence, you incurred an extended leave of absence from
March 28 until May 2,2023 without notice or communication to our office. This is not a simple
case of excessive absenteeism and/or unauthorized absences but tantamount to neglect of duty
and abandonment of work (specifically Category 4(y) in our Code of Conduct).
3. While you were not around and could not be reached, our production department suffered from
a lack of staffing that affected the overall productivity of the team.
4. While our company’s management was observing due process, you have been given more than
sufficient consideration by allowing you to make up for your shortcomings. However, you have
incurred more absences with unjustifiable excuses and/or without proper notice to our office.
These absences were considered as aggravating circumstances.
5. We must note and emphasize here that two schedules were arranged to conduct our
administrative hearings and you deliberately did not attend both. Thus, we had to proceed on
the second and final schedule on May 24,2023 with due respect to the time and attention
accorded to us by our administrative hearing panel members and our HR Consultant who
moderated and facilitated the meeting. We had taken your absence as an indication of your
“waiver” of your opportunity to be heard in the said meeting.
In consideration of all the facts regarding your case and the impact on our operations and business,
we are hereby rendering this notice of termination today, June 8, 2023 and to take effect
immediately.

Thank you for your service and we wish you the best in your future endeavors.
