AFFIDAVIT

I, RIZZA OMBAO, of legal age, Filipino, single and with residence and postal
address at Blk. 1, Lot 32, Ph. 2, Pluto St., Perpetual Village 7, Mambog V, Bacoor, Cavite
after having been duly sworn in accordance with law, hereby depose and state that:

1. When I started school sometime in 2001, I remember that I started using the name
“RIZZA OMBAO OLIVA;”

2. Sometime in 2018, I processed the late registration of my Certificate of Live Birth.


My full name in my Certificate of Live Birth is “RIZZA OMBAO;”

3. I interchangeably use the names: “RIZZA OMBAO OLIVA,” “RIZZA MAE


OMBAO OLIVA,” “RIZZA O. OLIVA” and “RIZZA MAE O. OLIVA” in my
official records, documents and transactions;

4. In fact, in my Secondary Student’s Permanent Record (Form 137-A) from General


Emilio Aguinaldo National High School, the name appearing on record is “OLIVA,
RIZZA OMBAO,” while my diploma from the same school reflects the name
“RIZZA O. OLIVA;”

5. In addition, in my Senior High School records from Southern Philippines Institute of


Science and Technology, the name appearing thereon is “RIZZA OMBAO
OLIVA;”

6. I am hereby executing this Affidavit to attest to the truth of the foregoing and to
attest to the fact that the names “RIZZA OMBAO,” “RIZZA OMBAO OLIVA,”
“RIZZA MAE OMBAO OLIVA,” “RIZZA O. OLIVA” and “RIZZA MAE O.
OLIVA” refer to one and the same person; and

7. I am executing this Affidavit for all legal intents and purposes.

AFFIANT FURTHER SAYETH NAUGHT.
