Ariel R. Mañacap declares in an affidavit that: 1) He is the biological father of Guinever D. Mañacap, born in 2005 in Cebu City. 2) He has allowed Guinever's mother, Mary Ann D. Kondo, sole custody since Guinever was 7 years old because he was unable to fully support her. 3) He freely and voluntarily consents to Guinever being adopted by Tomofumi Kondo, Mary Ann's husband, believing it would be in Guinever's best interest.

Affidavit of Intent and Consent


to Adoption of Child

I, ARIEL R. MAÑACAP, of legal age, Filipino citizen, and presently


residing at Sitio Zapatera Bo. Luz, Cebu City, after having been duly sworn
to in accordance with law, do hereby depose and say:

1. That I am the illegitimate father of GUINEVER D. MAÑACAP who


was born on 22 April, 2005 in Cebu City;

2. That I have allowed Mary Ann D. Kondo, the mother of the said
minor to have sole parental custody of our daughter GUINEVER D.
MAÑACAP since she was 7 years old;

3. That I was unable to provide full support for the said child and
believing that it would be for her best interest and benefit, I, hereby signify
my written intent, freely and voluntarily to have our daughter GUINEVER D.
MAÑACAP be adopted by Tomofumi Kondo the husband of her mother
Mary Ann D. Kondo;

5. That I am ready and willing, freely and voluntarily to abdicate my


parental authority over said child in favor of Tomofumi Kondo years old,
who possessed the necessary civil and legal rights as well as the financial
capacity to raise, rear and support my child, after the adoption proceeding
had been instituted and an order of adoption be issued;

6.That I am executing this affidavit to declare my intention and


consent to the adoption of my daughter GUINEVER D. MAÑACAP as well
as to the truth of the foregoing facts.

IN WITNESS WHEREOF, I have hereunto set my hand this
