AFFIDAVIT of DECLARATION OF GUARDIANSHIP

I, CHONA MARIE V. VILLARIN, Filipino, of legal age and a resident


of Pandacan, Pinamungahan, Cebu after having been sworn to on oath in
accordance with law, depose and say:

1. That I have in my custody my minor cousin, KENT JOSEPH


FAJARDO VILLARIN who was born on September 2, 2003 in
Pandacan, Pinamungajan, Cebu;

2. That she has been under my custody because his father Benito
Cuizon Villarin died sometime in 2020 while his mother Emma
Fajardo had abandoned him since his childhood;

3. That I undertake to take and declare custody of KENT JOSEPH


FAJARDO VILLARIN as his guardian due to the
incapacity/absence his parents;

4. That this affidavit is executed to attest and confirm the truth of the
foregoing statement and for whatever purpose this may serve.
