LESSEE, VENDEE, etc.] are collectively referred to herein as the “PARTIES.”

May it be known that the undersigned PARTIES, for good consideration, do hereby agree
to make the following change and/or addition to the aforestated Contract. This addition shall be
made valid as if it is included as an item in the terms and conditions of the original stated
Contract dated __________ as follows:

No other terms or conditions of the above-mentioned Contract shall be changed/modified


as a result of this Addendum.

The foregoing amendments/revisions to the Contract shall be effective
