I, Teresita O. Hernandez, of legal age, Filipino citizen, and


residing at No. 3057 Gen. Emilio Aguinaldo St., Brgy. South Cembo,
Makati City, after being duly sworn to, hereby depose and say:

That on _______________ 2018, I filed/served


three (3) copies of the “Protest” dated 20 July 2018 in the
case entitled “Teresita O. Hernandez vs. Rebecca
Nale”, personally to the Department of Environment
and Natural Resources-NCR National Ecology Center
and by registered mail, in accordance with Sections 3 & 5
in relation to Section 10 of Rule 13 of the Rules of
Court, by depositing a copy thereof in the
_______________ Post Office, in a sealed envelope, plainly
addressed as stated below as evidenced by the Registry
Return Receipts which is hereunder attached and
indicated after the name of the addressee and with
instructions to the postmaster to return the mail to the
sender after ten (10) days if undelivered:
