Greetings!

I am Adonis Hiramia Lambino a Regular/Permanent Elementary School Teacher of Deit de


Turag Elementary School at Brgy. Deit de Turag Silvino Lubos, Northern Samar. I am handling a
multi-grade classes (Grade 4 and grade 5).

In the interest of service, may I request your good office for a transfer of assignment from
Deit de Turag Elementary School to McArthur Elementary School of Las Navas District for valid
reasons stated herein based on the DepEd Order No. 22, s 2013.

1. My present station is far to my residence and has difficulty by means of


transportation. I will be more effective and productive teacher if my station is near to my
residence.
2. I am the head/bread winner of the family whom my siblings and my child
depend on me and I need to support them all by means because I am the only son who is
employed.
3. I want to pursue my Master’s Degree. For now, I cannot enroll because my
station is very far to the university, it takes more than 5 hours to travel.

Prior to my teaching experience, I have attended training and seminar conducted by the
District and Division Office.

With my knowledge, skills, experience and reasons stated above, I am hoping for your kind
consideration.
