and a resident of Blk 6, Lot 20, Gentiane. St., Camella Homes 1 Alabang Muntinlupa,
do to hereby NAME, CONSTITUTE AND APPOINT RODELYN D. CABRERA, also of legal
age, married, and a resident of Putatan, Muntinlupa to be my true and lawful attorney-
in-fact, for us and in our name, place and stead, and for my own use and benefit, to do
and perform all or any of the following acts and things, namely:

1. To represent me before Synchrony Financial in connection with the


processing of my final pay and SSS sickness check;

2. To receive and sign all necessary documents of my final pay and SSS
sickness check;

3. And to generally do and perform all and every acts which may be requisite,
necessary or proper to carry out the above purposes.

HEREBY GIVING AND GRANTING unto said attorney-in-fact full power


and authority to do and perform every act and thing whatsoever requisite and
necessary to be done in and about the premises, as fully to all intents and
purposes as I might or could do if personally present, hereby ratifying and

virtue of these presents.
