RE: FINAL DEMAND FOR PAYMENT AND NOTICE TO VACATE

Dear _________:

The undersigned writes on behalf of CLIENT’S NAME regarding the above-


referenced matter for the property you are currently occupying, located at
Magsaysay Ave. corner Saging St., Dadiangas South, General Santos City.

Under your lease agreement with our client, your monthly rental was supposed
to be paid every 30th day of the month1. Despite repeated demands, you failed to
pay and continuously refuses to pay your arrears which have now amounted to
_____________________________ (PhP ____________). For your
failure to pay, you have also accrued a penalty of ______percent (__%) of
the amount due.

Because of your breach, your right of occupancy has been terminated pursuant
to your lease agreement. However, your liability under said lease still subsists.

Due to the foregoing, you are hereby demanded to pay and to vacate the
property within five (5) days from receipt of this letter, otherwise we will
be constrained to immediately institute appropriate legal actions. We shall also
hold you liable for interest, damages, attorney’s fees, and expenses that may be
incurred.
