1. Project Summary:
In the modern world, most of the countries have modern automated systems for regular
proceedings especially in the health sector. In Pakistan there is still manual and
exhausting routine of prescription and patient’s data in hospitals when we talk about the
health sector. There is a dire need of the automated and computerised system in the
hospitals of Pakistan to keep track of the prescription of patients, disease and visit history
along with the record of patients. This is essential because carrying manual file of the
medical history is uncomfortable and most of the times it gets lost. To deal with this issue
a simple and efficient way of record keeping is essential that should be smart, available
and easy to carry. For this the proposed idea is to create a system that can keep the data
of the patient, prescription, medical history and visit history of when visiting the hospital.
The data would be kept saved and updated, every time a patient visits the hospital. The
patient could be identified for the retrieval of details through a card or unique identity
number. It will assure the privacy, security and the availability of patient’s record. In case,
if the patients forget to bring the card, the data can still be retrieved through the unique
patient identity number assigned to every patient. It will bring portability of medical
record as well as the emergency access to the patient’s record. This will bring ease for the
patient as well as for the health organization to treat the patient accurately and efficiently.
There is currently no system for the ambulance staff. In case of emergency ambulance
staff have to perform some medication at the spot. So, they don’t know about the previous
medical history of that patient. If a patient is having a problem like blood pressure then
what medicines are restricted by the doctor for that patient so that at time of first Aid they
avoid from that medicines. Also, there is no system for the medicine verification after
buying a chemist. Most of the people don’t know about that medicine either that is the
same one which is recommended by the doctor or not. So, there will be a mobile based
application for patient for the medicine verification.
2. Introduction:
In Pakistan, usually the hospitals are the health services providers and the health-related
issues are treated conventionally. A patient visits hospital and the doctor recommend
him/her the suitable prescription after check-up. The prescription is usually on the paper
and the patient has to keep it safe for the next visit. This is the current simple procedure
of the hospital. In the system being designed, the said procedure will be automated and
computerized for the sake of convenience for both the health organization as well as the
patient. It will also provide ease and efficiency along with the comfortable saving of the
record.

are used to write and scan the data to and from card respectively. The RFID card is
provided to the patient and the hospital will have the RFID scanner to scan the patient’s
card. On each visit the card is scanned for accessing details and the data against that
patient id is updated accordingly in the system. Using this modern method will save time
and it will also make the procedure of record management easy. The patient gets rid of
keeping the physical paper reports by this modern method. The authorized access
through the RFID card scanning will bring the security and privacy to the patient. The
centralized database provides the access of patient’s data globally that will enable the
patient to get medical facilities according to history around the world.
3. Project Aim:
The aim of this project is to build a web-based system that will provide ease of
proceedings with the security ensured. As well as a mobile based application for the users
to remind them about their appointments.
4. Project Objectives:
The objective of this simple project is clear and based on real worlds. The objective of the
system includes the automation and use of modern system instead of manual proceeding
so that efficient and easy process of carrying medical history. The objective is to
cumulatively focused to automate the normal routine of hospital in an efficient way.
Keeping the record in a computer system is another objective that provide the efficient
storage of record and accessing the patient medical history through a web forum. The
other objectives are to improve the quality of care and also to increase the administrative
efficiency. Mobile based application will help users to check their medical record
whenever they want. It will also help them to remind about their appointment.
5. Project Scope:
The scope of the project includes the web-based system for the doctors and medical staff
to check the medical history of a patient. Doctor can check his/her previous history and
also update or rewrite new information about their health and medicine. It will inform
patients about their new visit if doctor recommend them to visit the hospital after few
days. RFID tags will also be used for recognition of the patient and their previous
medical history. In our system we are using RFID cards for patients to avoid them
carrying their medical records whenever they have to visit any doctor. This will allow
them to not carry any medical record but a plastic card through which they can access
their previous medical records. There will be a mobile application for patient to verify
the medicine written by the doctor and for ambulance staff for customized first aid
description of a patient.

6. Problem Statement:
A patient visits the hospital and the doctor recommend him/her the suitable prescription
after check-up. The prescription is usually on paper and the patient has to keep it safe for
the next visit and also to carry all the medical history along him/her. This is the current
simple procedure of the hospitals or other health care centres. The main problem for a
patient is to carry all the medical records along with him/her. Sometimes they got lost.
Then it is difficult for doctor to recommend some medicine without checking their
previous history because in some cases patient cannot explain about their previous
condition. There is no inter hospital record sharing of a patient’s medical history. Also,
there is not specific information provided to ambulance staff for the basic First Aid of a
patient. Either they have to give him/her that medicine or not like if a patient is having
blood pressure then what medicines are restricted by doctors. Also, there is no
verification of the medicines after prescription of doctor. Sometimes chemist give patient
that medicine which are not written by doctor. If they don’t have that medicine, they give
medicine of same formula or according to them which will be suitable. And mostly people
don’t verify the medicine from doctor after buying from a chemist.
7. Proposed Solution:
We have proposed a web-based system which will resolve all the issues which currently exists
in health care centres with the help of RFID tags. RFID tags or uniquely assigned patient
identity number will be used to retrieve the patient’s medical history. We’ll automate all the
medical history of a patient so can he can access his/her medical history whenever he/she wants.
It will be very useful and efficient for both the health administrators as well as for patients also.
To overcome above problems, we are proposing centralized database from which all the people
can access data after performing some specific checks. And for ambulance staff there will be a
mobile application with patient’s limited data access. And for medicine verification there will
be mobile application from which a patient can check the medicine after buying from chemist
that this is the same medicine which is recommended by the doctor or not.
