AFFIDAVIT OF LOSS
OFFICIAL TRANSCRIPT OF RECORDS
AND TRANSFER CREDENTIALS

I, ABC, of legal age, Filipino, single, and a resident of _________, after


having been duly sworn to in accordance with law, hereby depose and state
THAT:

1) I was a former student of ___________ and sometime in 2018, the


Office of the Registrar issued a copy of my Official Transcript of
Records and a set of Transfer Credentials;

2) On or about January 2020 I noted that the said Official Transcript


of Records and a set of Transfer Credentials were missing;

3) To my knowledge the said documents were lost because I moved


from my Baguio Home to our residence in _____ and the documents
must have been mixed with other personal effects that has been
discarded;

4) I took great pains to look for the said documents but to no avail;

5) Despite diligent search and efforts to locate the said passport, I


could not find the same such that I now believe that they are now
lost beyond recovery;

6) I am executing this Affidavit of Loss to attest to the truth of the


foregoing and to support the application for the issuance of new set
of Official Transcript of Records and a set of Transfer Credentials in
lieu of the one which were lost.

IN WITNESS WHEREOF, I have hereunto set my hand this 14th day of


January, 2020 at Baguio City, Philippines.
