1.0 OBJECTIVE

The objective of this ERP are:

1. To provide a prompt and coordinated response during the unexpected event of an


emergency for the protection of people, property, environment and general public
2. To provide procedures to be adhered to and define the responsibilities and actions to be
taken by the designated personnel during an emergency
3. To ensure an effective mode of communication exist among personnel coordinating various
tasks while managing an emergency as lack of co-operation will impair any response plan.

2.0 SCOPE
The scope of this procedure covers any potential emergency/crisis situation that may occur in
the Project Phase 5C

5.1 Activation- The start of an emergency response during the emergency.

5.4.1 Crisis - Crisis is when a situation that happened could


contribute to a significant business disruption, which will
result in an unstable condition in a company’s business and
attract extensive media coverage. The resulting public scrutiny
will affect the organization’s normal operations that could have
legal, financial or other impact on its business. Page 1

5.5 Debriefing- A short meeting let by the ERT Controller after the emergency
5.6 Direct Actions- Actions to reduce and eliminate the threats of
the emergency such as fire fighting, first aid, search and rescue,
etc

5.7 Emergency- Any unwanted (identified) incident that may


endanger the people or property

5.8 Emergency operation centre- A command centre


during/prior to the emergency, equipped with the

responding to the emergency

5.9 ERT/EMT/CMT Controller- the responsible person


for the overall control of the emergency responses and
coordination with relevant squads and parties in order
to contain the emergency situation. In his absence, the
deputy shall take over the responsibility.

5.10 Emergency roles- Functions and activities the


need to be performed and fulfilled for the emergency
response.

5.11 First responder- the person who first discovers


and responses to a potential or an actual emergency

5.12 Levels of emergency- The degree of danger and


threat of the emergency to people, Property,
environment, or business operation.

5.13 CMP – Crisis management plan

5.14 Termination of emergency- End of emergency and


declaration that it is safe for people to return to their
normal duties

6.0 EMERGENCY RESPONSE TEAM


6.1 Setting up of ERT

The project shall establish a team to handle and manage the potential emergency
situation.
The team shall, as far as practicable, comprise of the following;

ERT Composition

ERT Controller

Deputy ERT Controller

Damage Control Coordinator

Logistics Coordinator

Communication Coordinator

Evacuation Coordinator

Search & Rescue Coordinator

First Aid Coordinator

The selection shall be based on the members’ Level of authority and capability. The
actual composition of ERT shall be decided based on the actual number of staff available
at site. The project’s ERT Organization is as per Appendix III

6.2 Drill and Review


The ERP shall be tested at least once a year as directed by the authorized
personnel/department. The testing is by mock drill is to determine the effectiveness of
the emergency response team in handling such emergency situation. The adequacy of
equipment, readiness of ERT and resources shall be among the criteria to be assessed.
The result of the drill shall be documented and shall be kept at the workplace. The ERP
will be reviewed after any occurrence of an emergency situation by the workplace
management. During the revision, discussion may be held between the ERT members or
seek expertise from outside agency where arrangement would be made accordingly.

7.0 TASK AND RESPONSIBILITY


The workplace person in charge, who is automatically the emergency response team controller,
shall be responsible to lead the team in preparing, communicating, implementing and reviewing

the ERP.

The details of task and responsibility of the emergency response team are describes in
Appendix iv

8.0 PROCEDURE
8.1 Activation of the ERT

The emergency response team is activated as soon as any site employee becomes aware
of an emergency arising within the workplace. Every person in the workplace must be
aware of each alarm and its indication.

Activation of ERT shall start once the ERT controller receives the notification of
emergency and he confirms it as an emergency

In his absence, the confirmation of emergency shall be decided by his deputy or any
other authorized personnel.

8.2 Emergency Response


Appendix V shown an overall emergency response flowchart. When an alarm is
triggered during an emergency, up to two response centers could be set up, depending
on the level of emergency as determined by the ERT controller. Where external
resources are needed, the emergency services such as fire and rescue department,
police, etc. shall be immediately notified for assistance. When the fire and rescue
department arrives, the lead officer takes control of the emergency response and the
ERT controller shall assume an advisory role.

8.3 Communication flow


The communication flow between emergency response team, Emergency management
team and crisis management team is shown in Appendix VI

8.4 Emergency Operation Center (EOC)


An emergency Operation centre (EOC) shall be established at the workplace.Page 4
During
actual emergency situation, all members shall report to their duty to the ERT controller
or deputy ERT controller at the EOC. The EOC shall also have the communication
facilities to enable the ERT communicate with relevant parties or authorities.

Section 3.2 – Project Brief and Section 3.3 – Project Location.

8.6 Levels of Emergency

The levels of emergency shall be categorized into three levels based on the criticality of the
emergency situation itself.
Emergency Level 1 shall activate Emergency Response Team only.
Emergency Level 2 shall activate Emergency Management Team.
Emergency Level 3 shall be called a Crisis and will require the activation of Crisis Management
Team.

The classification of emergency level is described in Appendix VI of Crisis Management Plan.

8.7 Resources and Facilities

8.7.1 The Resources and Facilities of workplace Emergency Equipment Log refer to
(Appendix IX)

8.7.2 First Aider


The workplace shall maintain adequate first aid facilities at side to provide
treatment for injured person. First aid provision shall be as required by legal
requirements. Only trained First Aider shall conduct first aid treatment. The first
aid activity shall be under the control of the First Aid Controller. The first aid
facility such as first aid kit, stretcher and first aid room shall be located at an
accessible location.

8.7.3 Fire/Explosion Protection System

a) Adequate numbers of portable fire extinguisher shall be provided at strategic locations
and accessible by all persons at site to facilitate the fire fighting activity.
b) Fire extinguisher shall be clearly marked with proper signage.
c) ABC Dry Powder and CO2 type fire extinguisher shall be provided.
d) Access to fire extinguisher shall always be maintained free from obstruction.
e) These fire extinguishers shall be inspected regularly and recharged every 12 months or
after use to ensure that they are always in good working condition.
f) Defective fire extinguisher shall be removed immediately from the site and replaced.

Emergency Siren

a) Adequate numbers of emergency siren shall be located at site to warn employees during
emergency situation.
b) These sirens shall be located at strategic locations, which will be clearly audible to all
personnel at site.
c) These sirens shall be periodically tested to ensure there are always in good working
condition.
d) The switches shall be positioned at appropriate locations, where they are accessible.

8.8 Assembly Area


The Person In Charge is responsible to identify a suitable Assembly Area with the following
requirements:-

i. Allocation of an area having adequate size to accommodate the numbers of site


employees.
ii. The area shall be clearly marked with proper sign board.
iii. The area shall be kept free from any obstruction.
iv. Layout plan showing the evacuation route from work areas to the assembly area shall be
establish.

The layout location of Assembly Area is shown in Appendix VII.

8.9 Immediate Action during Emergency


8.9.1 Person Injury or Sudden Illness

a) The first person at the scene shall notify the ERT Controller via phone call or any
other suitable method.
b) The emergency contact numbers shall be made available at workplace. The
emergency contact numbers shall be as per Appendix VIII.
c) Upon receiving the notification, the ERT Controller will summon the First Aider to
the area where the injured person is reported.
d) If the injury is minor, the injured person shall be brought to the first aid room to get
his treatment.
e) The First Aider will give first aid treatment to the injured person and decide whether
an ambulance is needed.

f) If the injury is serious and injured person could not be moved, the First Aider will call
for ambulance.
g) If necessary, the work area shall be evacuated to prevent further accident. The work
area shall be cordoned to prevent another person from entering the area.
h) If the injured person is working for a sub-contractor, the ERT Controller shall inform
his employer regarding the situation.

8.9.2 Fire and Explosion


The action to be taken during the fire shall vary based on the category of fire.

a) Any person who discovers the fire shall try to extinguish the fire using the available
fire fighting equipments provided at site.
b) Once eliminated, the person shall inform the ERT Controller of the incident.
c) The ERT Controller shall summon the Damage Control Coordinator to the fire area to
access the situation.
d) The Damage Control Coordinator shall report to the ERT Controller on the situation
of the fire.
e) If the fire is large, the person who discovers the fire shall immediately active the fire
alarm/siren provided.
f) He shall then report to the ERT Controller by phone or by other suitable methods.
The emergency contact number shall be displayed at workplace.
g) He shall evacuate the area and proceed to the assembly area.
h) ERT Controller shall notify Damage Control Coordinator and his squad to access the
fire situation.
i) Damage Control Coordinator with his squad shall verify the scale of the fire and if
possible will take action to control or confine the fire.
j) At the same time, the ERT Controller shall notify the Fire and Rescue Department for
external help.
k) Once the siren is activated/evacuation order is given by the ERT Controller, all
workers and visitors must evacuate the work area and proceed to the assembly area.
l) The Evacuation Coordinator with the help of trade supervisor will guide the
employees during the evacuation process.
m) At assembly area, site employees are to gather according to their work group/trade.
n) Evacuation Coordinator shall proceed to the assembly area and report to Page the ERT

Controller once all site employees have been evacuated.
o) The Communication Coordinator with the help of trade supervisor shall verify that
all site employees are present by conducting roll call.
p) Any missing person shall be identified in which the Search and Rescue team will be
sent to search for the missing person if the situation permits them to do so.
q) The First Aid Coordinator with his team will stand by at the assembly area to give
first aid treatment to any person when required.
r) If the situation does not permit the search and rescue team to enter the building,
the Communication Coordinator shall wait for the Fire and Rescue Department to
arrive and inform them of the missing person.

s) Logistics Coordinator shall ensure that all access to site is accessible and shall guide
the authorities into the site.
t) All employees shall be required to remain at the assembly area until the ERT
Controller issues the all clear order.
u) ERT member shall be at the assembly area to aasist the authorities.
v) When the fire is totally extinguished the ERT Controller after consulting the Fire and
Rescue Department will give the all clear order to all employees.
w) The permission to enter the building will be based on the permission given by the
authorities.
x) In the event of permission to enter the building is not granted by the authorities, the
employees will be instructed to leave the work area.
y) The ERT Controller shall record all the particulars of witnesses and injured person.
z) The injured person (if any) shall be sent to the hospital by ambulance or any suitable
vehicle.
aa) Communication Coordinator shall follow up with the hospital on the condition of the
injured person.
bb) If the injured person is working with a sub-contractor, the Communication
Coordinator shall liaise with the sub-contractor on the medical status of the injured
person.
cc) The copy of the passport/identify card of the injured person shall be made available.
dd) No other personnel shall be allowed to enter the building without approval.

8.9.3 Dangerous Occurrence


a) Any person who discovers the situation shall report to the ERT Controller by phone
or other suitable methods. The emergency contact number shall be displayed Page 10at
workplace.
b) He shall then evacuate the area and proceed to the assembly area.
c) ERT Controller shall notify Damage Control Coordinator to assess the situation.
d) Damage Control Coordinator with his squad shall verify the seriousness of the
situation and check for the possibility of trapped person.
e) If the situation is minor without any case of injury and will not have the potential to
cause any further damage/injury, he should barricade the area and report to the ERT
Controller.
f) ERT Controller will decide whether evacuation is needed.
g) In the event evacuation is needed, emergency alarm will be activated by Damage
Control Coordinator.

h) At the same time, the ERT Controller shall notify the external emergency services for
assistance.
i) Once the siren is activated/evacuation order is given by the ERT Controller, all
workers and visitors must evacuate the work area and proceed to the assembly area.
j) The Evacuation Controller with the help of trade supervisor will guide the employees
during the evacuation process.
k) At assembly area, site employees are to assemble according to their work
group/trade.
l) Evacuation Coordinator shall proceed to the assembly area and report to the
Communication Coordinator once all site employees have been evacuated.
m) The Communication Coordinator with the help of trade supervisor shall verify that
all site employees are present by conducting roll call.
n) Logistics Coordinator shall ensure that all access to site is accessible and shall guide
the authorities into the site.
o) All employees shall be required to remain at the assembly area until the ERT
Controller issues the all clear order.
p) ERT member shall be at the assembly area to assist the authorities.
q) ERT Controller shall give the all clear order after consulting the related authority.
r) The permission to enter the building will be based on the permission given by the
authorities.
s) In the event of permission to enter the building is not granted by the authorities, the
employees will be instructed to leave the work area.
t) The ERT Controller shall record all the particulars of witnesses and injured person.
u) The injured person (if any) shall be sent to the hospital by ambulance or any suitable
vehicle.
v) Communication Coordinator shall follow up with the hospital on the condition of the
injured person.
w) If the injured person is working with a sub-contractor, the Communication
Coordinator shall liaise with the sub-contractor on the medical status of the injured
person.
8.9.4 Chemical Spill

a) Any person who accidentally spills any chemical or spots any chemical spillage 11
notify the nearest supervisor. The supervisor must immediately rush to the location
and assess the situation:-
i. Confirm if any person is involved.
ii. Make arrangement to contain the spillage.
iii. Cordon off the area concerned.
iv. Request for help from another employee.
v. Inform the ERT Controller.

b) Where people are involved, the supervisor at the location must make attempts to
save the person involved. This may involve, depending on the situation and the
chemical involved, flushing the person concerned with a lot of water, removal of
contaminated clothing or making arrangement for medical aid.

c) The supervisor at the location must initiate containment and preventive action to
mitigate the impacts associate with the chemical spillage. The action may include,
but not limited to the following:-
i. Use suitable absorbent materials to absorb the chemical.
ii. Where the spillage tends to flow to other areas, attempt to stop the flow by
putting absorbent materials where necessary.
iii. Where the supervisor determines that the situation is out of control and he is
unable to contain the spill, the supervisor must seek help from external agency
with the approval of ERT Controller.
iv. The relevant outside agency to be contacted may be BOMBA, chemical
manufacturer etc.
v. Never touch the chemical and contaminated area, absorbent materials with bare
hands.
vi. Use scoop to remove the contaminated rag, absorbent materials and put the
same into a container.
vii. Label the container as toxic substance and transfer it to the Scheduled Waste
Storage Area

d) Designated person of a respective workplace shall ensure that the absorbent


materials have been replenished after each occurrence of emergency situations.

8.9.5 Land Slide/Slope Failure

a) Any person who discovers the situation shall report to the ERT Controller by phone

or other suitable methods. The emergency contact number shall be displayed at
workplace.
b) ERT Controller shall notify Damage Control Coordinator to assess the situation.
c) Damage Control Coordinator with his squad shall verify the seriousness of the
situation.
d) If the situation is minor without any case of injury and will not have the potential to
cause any further damage/injury, he should barricade the area and report to the ERT
Controller, who will take the necessary action to rectify the failure.
e) In the event of employee(s) or any other person being buried under the earth, the
ERT Controller shall notify the external emergency services for assistance.
f) ERT Controller shall notify EMT Controller if the emergency situation falls under
Emergency Level 2 or 3 categories.

g) The ERT Controller also shall decide whether evacuation is needed. In the event
where evacuation is needed, the alarm shall be activated and avacuation process to
assembly area will take place.
h) Logistics Coordinator shall ensure that all access to site is accessible and shall guide
the authorities into the site.
i) All employees shall be required to remain at the assembly area until the ERT
Controller issues an all clear order.
j) ERT member shall be at the scene of the emergency to assist the Fire and Rescue
Department if required.
k) ERT Controller shall give an all clear order after consulting the authority concerned.
l) The permission to restart work will be based on the permission given by the
authorities.
m) In the event of permission to restart work is not granted by the authorities, the
employees will be instructed to leave the work area.
n) The ERT Controller shall record all the particulars of witnesses and injured person (if
any).
o) The injured person (if any) shall be sent to the hospital by ambulance or any suitable
vehicle.
p) Communication Coordinator shall follow up with the hospital on the condition of the
injured person.
q) If the injured person is employed by a sub-contractor, the Communication
Coordinator shall liaise with the sub-contractor on the medical status of the injured
person.

8.9.6 Flood/Silt Trap Failure

a) The first person at the scene shall notify the ERT Controller via phone callPage 13
or any
other suitable method.
b) Upon receiving the notification, the ERT Controller will summon the Damage
Controller to the area where the flood/slit trap failure is reported.
c) The Damage Controller shall take the necessary action to mitigate further damage
and minimize impact due to the event.
d) Emergency equipments and machinery will be mobilized to the affected area to
rectify the damage.
e) In the event where water overflows into public boundary, the Logistics Controller
shall conduct an assessment on the magnitude of the damage.
f) Clearing of debris or residue due to the overflow shall be carried out immediately.

8.10 Termination of Emergency

The ERT Controller is responsible for termination of the emergency. Appendix IIX shows the
emergency termination procedure. Termination shall take place when the emergency response is
completed and the emergency situation is over. When it is safe for the people to return to
normal duties, the ERT Controller would announce the termination of emergency by declaring:-

“ The emergency is over. All personnel return to normal duties.”

8.11 Action after Termination of Emergency

When the emergency has been terminated, the ERT Controller is responsible for assessing the
emergency response as well as proper reporting of the emergency to the relevant department in
the company and government agencies.

8.11.1 Review of Emergency Event

Review of the emergency shall be conducted after the emergency has been terminated.
The review shall be led by the ERT Controller. The following is addressed during the
review session:-

i. History of the event (how the emergency started, if known)


ii. Actions taken during the emergency
iii. Safety measures to be taken after the emergency
iv. Reports on the number of injuries and fatalities during and as a result of the
emergency
v. Follow up activities
vi. Feedback from the emergency response team
vii. Recommendations to prevent future recurrence or to improve emergency
response
viii. Discuss the adequacy of the ERP and available emergency equipment, and
whether modifications are necessary Page 14

8.11.2 Reporting

Reporting shall be in line with Incident Management Procedure. A comprehensive report


must be prepared to identify the immediate and basic cause(s) of the emergency. Each
member of the emergency response team is responsible for keeping his/her own log.
The ERT Controller is responsible for compiling all logs and assist Safety and Health
officer on preparing the report.

8.12 Media and Public Response

No workplace personnel shall give any comments or statements to the media or public on the
emergency without authorization from Crisis Management Team Controller.

Only designated spokesperson(s) from the Crisis Management Team shall handle any comment
or statement to media and/or public.

8.13 Document Control, Update and Review

The Person in Charge shall be the document controller for the ERP document. He/she is
responsible for the maintenance, amendment and review of this ERP.

Appendix (IV)

ERT TASK AND RESPONSIBILITY

1) Emergency Response Team controller

The ERT controller is in change of coordination between the on site emergency operation center
and the crisis management team at head office. He will call for evacuation when needed. The
workplace person in charge will take a position as the ERT controller. In his absence, the
construction manager or other authorized personnel will be the deputy ERT controller.
The ERT controller has the following responsibility:

a) Decide on the level of emergency and if an evacuation is required, as well as determine the
need for EMT or CMT is to be activated.
b) Decide on site evacuation
c) Specify evacuation assembly area, where necessary, and ensure that all personnel are
familiar with its location. If the primary assembly area is not accessible during an emergency,
an alternative location shall be designated.
d) Advise the communications coordinator to make the necessary call outs and notifications.
e) Examine the emergency to determine the source
f) Take control of the situation until relieved by an emergency services officer (fire and rescue
department takes control of the emergency response, while the police takes control of the
access and security of the workplace) at which point the resumes an advisory role.
g) Issue orders for appropriated team members to initial all intervention and protection system
(containment, isolation, traffic control, fire fighting, search and rescue)
h) Assess damages and casualties
i) Resumes control when the site is handed back from the emergency services officer
j) Debrief and makes appraisals of performance when the emergency is over
k) Record all event and action taken
l) Report to EMT controller on overall process

2) Deputy ERT controller


The deputy ERT controller shall assist the ERT controller and in the absence of ERT controller, he
shall carry out the tasks and responsibilities of ERT controller

3) Damage control coordinator


The damage control coordinator shall lead the following;

a) Notify and assemble ERT members


b) Lead his team to fight fire, contain spills, cordon off emergency area and other mitigation
actions as required depending on the emergency situation
c) Report to ERT controller on the overall process

4) Logistics coordinator
The logistics coordinator shall lead the following;

a) Organize facilities and equipment for ERT


b) Maintain access and egress to be free from any obstruction
c) Guide the authorities to the emergency scene once they arrived
d) Record all actions takes during the emergency for compilation
e) Report to ERT controller on the overall process

5) Communication coordinator
The communication coordinator has the following responsibility:

a) Assist ERT controller to contact ERT members


b) Liaise with the emergency services, i.e ambulance, fire and rescue department, police, and
other authorities under the directive of the ERT controller.
c) Record all communications during the emergency
d) Perform head count with the assistance of respective supervisors
e) Acts as log keeper for compiling all records and logs from each emergency coordinator
f) Report to ERT controller on the overall process

6) Evacuation coordinator
The evacuation coordinator has the following responsibilities:

a) Activate evacuation team members once instructed by the ERT controller


b) Ensure all parties at workplace evacuates the work area
c) Communicate with the communication coordinator once evacuation process is completed
d) Report to ERT controller on the overall process

7) First Aid coordinator


The first aid coordinator has the following responsibilities:

a) Organize and arrange first aid kits


b) Provide first aid treatment to the injured person(s), if any
c) Report to ERT controller on the overall process

8) Search and rescue coordinator


The search and rescue coordinator has the following responsibilities. However, the search and
rescue shall only be carried out if the situation is safe to do so. In the event of the situation is too
dangerous and does not permit the search and rescue to be carried out, the search and rescue
coordinator and his team will stand by at the emergency scene and assist the external agencies
