2. OPERATIONS:

NP, NBI, DI Clearance & SSS Nr?
X 6 Are all the SGs issued MO/DDO prior to posting or dispatch? How
long the duration of the order? Monthly.
X 7 Does the agency maintain a list of clients?
X 8 Does the agency publish daily disposition of guards, firearms and
clients?
X 9 Does the PSA provide armored vehicle services?
X 10 Does the agency has communication facilities and what? Handheld/
Base, Landline/Cellphone How many? 1 landline 5 cellphones
X 11 Does the PSA conduct troop information and education to their
guards? If so, how often?
X 12 Does the agency establish a policy conducting information to the

3. TRAINING

X 10 Is the Program of Instructions (POI) is in accordance with the
prescribed rules by the PNP SAGSD?
4. INTELLIGENCE
X 1 Is the Optr? Proprietor natural born citizen?
X 2 Is the PSA a corporation? Does it has an incorporator who is not a
natural born in the country? How many? Pls list down their names.
X 3 Is the agency has an intelligence gathering plan?
X 4 Is there an intel section or personnel handling intel matters?
X 5 Are their investigators employed in the agency?
X 6 Is the agency maintaining a rogue’s gallery of wanted criminals
within their area of opns?
X 7 Is the management closely coordinating with the PNP Units within
AOR? If so, how?
X 8 Is the agency furnishing after Investigation Report to PNP SAGSD
they have conducted?
X 9 Is the agency maintains a file of dismissed or discharge/terminated
SGs that were involved in some cases while still employed with
them?
X 10 Is the agency coordinating with other PSA re deployment of security
guards dismiss?
X 11 Is the agency capable of conducting intelligence surveillance
mission?
5. LOGISTICS
X 1 Is the collection of clients fee for security services on a monthly
basis? If not, how?
X 2 Is the agency providing/giving at least one (1) free set of SG
Uniform and paraphernalia for every guard employed per year? If
not, how ?

X 3 Does the agency has enough stock of supplies and materials for
use of their operations?
X 4 Does the agency allocates an amount for POL or has a depository
of gasoline to a certain gas station for their service vehicles? If not,
how does it support their inspection team?
X 5 Are there agency’s company’s firearms stock at the firearms vault?

6. GUARDS ON POST
X 1 Do all guards inspected valid PSL to exercise profession?
X 2 Are the security guards uniform is in accordance with the prescribed
set of uniform?
X 3 Are they familiar/proficient in the provisions of Conduct and other
rules and regulations?
X 4 Are all security guards complying with the prescribed hair cut?
X 5 Are the guards have in their possession valid firearm licenses, duty
detain order?
X 6 Are all the guards familiar of the environment wherein they are
posted?
7. COMMENTS/FINDING:
FOUR “A” Security Agency Security Agency has completed all the requirements
for Renewal of their License To Operate (LTO) Private Security Agency.

8. RECOMMENDATION:
Recommend for the approval of their application for RENEWAL to operate a
Private Security Agency.
